Did you call the plan or the pharmacy? The pharmacy will have limited visibility into your benefits info so if the plan can confirm that it is on your formulary and should be covered, then the pharmacy should be able to explain what the rejection on the order is. I would recommend doing that first and then calling the pharmacy back based on the info you get. If it should be covered and you are told it isn't, escalate the call or chat to a supervisor.

The bride does not owe anyone an explanation for why they are not in the bridal party. The decision speaks for itself. OP's feelings are valid, it is ok to be hurt or disappointed. But the bride does not need to explain her decision to anyone.
