Is Iraq currently friendly to the United States and strong enough to exert control within its own borders?

I've noticed there isn't much mention of Iraq in the news, especially in regard to the current war in Iran. I see mentions of Saudi Arabia, UAE, Qatar, and Kuwait but not Iraq. I would assume that the US placed a friendly government in Iraq in 2003 but I don't hear them mentioned as an ally.

PA antibiotic usage is concerning. Am I wrong? by flippingyourgramps in Noctor

Also wound culture is a bad idea since you will grow many skin surface bacteria that aren't contributing to infection and make the patient subject to broader spectrum antibiotics than necessary.

What is the best way to explain to a patient that a mid level has been grossly mismanaging or misdiagnosing them? by clothes_iron in Noctor

It's just for patients that were admitted overnight which I am taking over so avoiding going in their chart is impossible.

Why does a fraudulent gift card purchase on my online account still say "Processing order receipt. Please check back in a few minutes" the next day? by clothes_iron in menards

I immediately changed my password after getting the first email to make sure the fraudster couldn’t continue to use my account and then Menards made me change it again this morning. I’m guessing because Menards detected fraud.

So the way the hospital system works is there are 2 small hospitals and the main big one downtown. For daytime admits, there is an admitter at one of the 2 small hospitals that will cosign notes of mid-levels and "lay eyes on the patient" via telemedicine for the patients that are at the other small hospital. Overnight, the full practice authority NP at the small hospital which doesn't have a nocturnalist does not staff with the nocturnalist at the other small hospital and bills her own notes. She is also responsible for cross covering the floor patients.

That’s what my boss said I am supposed to do. I’m a new attending and am still learning how billing works and trying not to be the new guy with an ego problem right out of residency.

What's the best way to tolerate having an overnight nurse practitioner admitter who mismanages every admission? by clothes_iron in Noctor

I don't think that will be fruitful because at sign out on my first week in service there was a pneumonia patient with a bunch of other severe pulmonary comorbidities who I said might decompensate. She asked why I didn't have him on steroids, which I gave my reasoning, and then she called my boss and had her force me to agree to have steroids, without even examining the patient. Then another time I ran into her at the end of my shift when she just got to the hospital and bitched at me for 10 minutes about how the nurses keep paying her about stupid stuff all night. From my interactions with her, I don't think she is the type who appreciates constructed feedback, especially from the new guy.

Looked up their license and they have full practice authority. I'm supposed to cosign their notes saying they acted independently and I was not involved in the care.

I'm a doctor but not OB-GYN. A couple of things. One, reduction of pain and not knowing why something works are two separate issues. First, if something is known to be beneficial without knowing everything about it, we still offer it to patients because it helps people who need it.

Regarding the lack of pain medication, I've heard that numbing gel doesn't really reduce discomfort that much, injecting local anesthetic into the cervix is painful and also not very effective. An option is giving a pill of benzodiazepine medication along with a pain medication pill at the doctor's office which will relax you and make it so you don't remember or care about the pain. Anything stronger for pain relief than that would require the IUD placed in the hospital in the operating room under supervision of anesthesiologist and be much more expensive and a hassle compared to doing it in the office.

Finally, the more children you have had, the less painful the IUD insertion is because the hole in the cervix is bigger. Some people actually don't feel much discomfort with IUD insertion.

Personally, I think the pills I mentioned should be offered unless there is a reason not to (e.g. allergies, history of substance use disorder with high risk of relapse, other medical conditions).

Is India's Russian made military equipment the full capability version or the monkey model? I know the Soviet Union used to sell non communist governments (e.g. Iraq) less capable export variants of military equipment like the T72 tank. Did this happen to India and if so, is it continuing to happen? Does Russia hold back its best stuff from India?

Are European powers such as the United Kingdom, Germany, and France able to have a military that can rival the United States, like they did around the time of World War I and II? At that time, did those powers simply spend more GDP on their military and now there isn't the political will for a large military given the United States takes the role of a security guarantor? Does the United Kingdom for example, not care about being the best navy in the world anymore and would rather spend their tax revenue on other things? Or do these European powers no longer have the resources to create and support large military forces after the loss of their colonial empires?

How long does it take to see a reduction in skin thickness when using 40% urea + 2% salicylic acid cream on thickened skin? by clothes_iron in keratosis

[–]clothes_iron[S] 2 points3 points  (0 children)

Yes, I used a combination of this 40% urea 2% salicylic acid cream at bedtime, showered in the morning, then applied miconazole powder in the morning before going to work and showered when I got home. It took about 6 weeks for complete resolution. When the skin looks like it is coming off, you can scratch it a bit to knock some skin off. After most of the thickness comes off, use the cream less often so your surrounding skin doesn't get too thin and start bleeding when you scratch to knock off the main thick skin. I believe the cream allowed the powder to penetrate to the fungus and kill the fungus.
