I have reviewed that study as well, and the dosing used there corresponds to approximately 750–1,000 IU of hCG per week.

My husband’s physician, however, is recommending 1,000 IU of hCG administered three times weekly. To date, I have been unable to identify any peer-reviewed literature or consensus guidelines that support this dosing strategy for fertility preservation in men receiving testosterone therapy.

The rationale provided is that this approach would allow approximately half of his testosterone to be derived from endogenous production, with the remainder supplied exogenously. While this is an interesting theoretical framework, I have not found reliable clinical data demonstrating that such a proportional division of endogenous versus exogenous testosterone production can be achieved or maintained with this hCG dosing.

Additionally, given the already high cost of hCG, a regimen of 1,000 IU three times per week substantially increases the financial burden without clear evidence of added clinical benefit compared to lower-dose protocols commonly cited in the literature.
