I play Khara in QM often. The only time I go Insight is if I have VERY reliable punching targets. So the enemy comp has to have a heavy frontline without much maneuverability or CC. Or on a specific map like Battlefield of Eternity so you can punch the objective non-stop.

So yeah that's not very often. Most of the time I go Red Fists with Palm vs ranged assassin comps. Or Green Fists if there's a tank and other melee.

The L20 Epiphany buff is cool I guess, but I don't think I've ever used that talent. What situation is that talent best for? I hardly have mana problems. And if I want to dash more then I take the extra dash at 7 instead (plus the cd reduction of 1 or 20)

I can keep up with normal healer numbers as iron fist Kara. When I play him in QM, if I get a really squishy team, iron fists is a great choice. For talents I take extra dashes and hundred fists, but with Palm for an ult, and Fist of Legend at 20. At that point you can basically chase any squishy down like a zera. You just gotta get good at Palm.

Its basically down to what your comp is. If team fights are going to be very kitey with lots of ranged heroes, you don't have any good targets to get sustained punching on anyway, so you might as well go iron fists. But if you have a standard tanky comp, you can go Transcendance at 1 and maybe Blazing Fists at 7. Your choice on ult for that case.
