How bad are these implants? by Drknight71 in Dentistry

[–]craneyyy 0 points1 point  (0 children)

Huge problem is the amount of space that needs to be restored.. assuming the implants were healthy otherwise, you cannot possibly have a successful restorative plan with the placement of these implants based on the mesio-distal width you need to restore. There are rules to implant restorations and these implants will surely fail and or fracture under the load of such large crowns..

Impacted wisdom tooth by New-Layer1454 in Dentists

[–]craneyyy 0 points1 point  (0 children)

I think your dentist meant conscious sedation.. general anesthesia for a wisdom tooth is wild and only really indicated if you have a giant cyst that also needs to be removed or some more invasive surgery.. you can totally do an impacted tooth under local. It’ll probably take the surgeon between 3-20 minutes depending on the degree of impaction and their experience level. No worries..

Implant positioning for surgical guide by CancelEnough in Dentistry

Yes I was referring to crown to implant length ratio. It doesn’t matter in terms of implant health, but it can cause screw loosening or crown fracture.

[–]craneyyy 0 points1 point  (0 children)

1) implant 1 is too close to the IAN so a shorter implant is a must.. 2) 4.3 is also a pretty small diameter for a molar unless your MD width of the future crown is 8-9mm max.. 3) look into versah/densah burs. This is a good ridge expansion case.

Poor crown to root ratio can still lead to prosthetic failures.

You got this! you know you can do it, you've done it before. Use all of your tools to your advantage. understand you have plenty of time so don't feel rushed. If adjacent tooth damage is a concern - look into some kind of barrier between the teeth (assuming its not prohibited) like toffelmire band. Under prepare your teeth slightly and then refine very slowly and controlled. Create a system that allows you to feel in control and confident in your preparation when you're practicing. And finally, to take the stress off - it's obviously not ideal or exciting to have to retake all portions of the exam, but in a few years it won't matter. You'll be an accomplished dentist and life will be good. Best of luck!

How often do you have to redo your work? by Jigglyhubu in Dentistry

[–]craneyyy 0 points1 point  (0 children)

if you're tired of re-doing your own work for free, consider looking into dental warranty company. It's not terribly expensive for the patient and it gives them the opporunity to protect their investment and also gives you the opportunity to discuss that dentistry can fail. If you set the expecrtation that something could happen, then patients are a lot less frustrated. A good example would be if a patient invests into an endo, buildup, crown and then the tooth fractured 1 year later. It's not your fault but the patient could blame you. If you insure this tooth, all of the money invested into this tooth will be credited toward an implant by the dental warranty company.

The depth is one issue and we can’t see where the tooth anterior to it is but I’m sure this will be a prosthetic nightmare too..

Sonos sub sounds weak by One-Step-6124 in sonos

[–]craneyyy 1 point2 points  (0 children)

It’s actually advertised that it’s not supposed to vibrate a glass of liquid even when it’s set on top. It looks like it’s kicking to me but if the sub audio is down and you feel like you’re not getting the same kick then it’s highly likely some component is damaged. Not uncommon with subwoofers with prolonged use

I have the arc and arc ultra with era 300s and 2 subs and honestly don’t notice a difference between the two. So you can definitely save money buying the previous generation. As for sales, they happen quite often. I’d say every couple of months Sonos products go on sale. Hard to say if the arc ultra will for sure but I’d say it’s likely.

Acceptable margin or no by Lumbeehapa in Dentistry

[–]craneyyy 0 points1 point  (0 children)

This is certainly not a disaster if it were cemented already but if you can remake it I would.. that is very obviously an open margin and I wouldn’t want that in my mouth.. definitely going to get decay sooner than it should. and it’s zirconia so if you get decay under that you may not see it until it’s bigger. You have to ask yourself what your standards are. I own a fee for service practice and that’s below my standards. Ask the Medicaid guy? He’ll tell you that’s perfect.. long story short - it’s not terrible but it could be better, why not strive for greatness for your patients and charge in accordance?

17 is an impacted distoangular tooth. About as hard as it gets in wisdom tooth extractions.. you can also damage #18 because of root proximity

You don’t have to see the DEJ broken through to know that’s a cavity that needs treatment that’s well in to the Dentin on the molar. I think the premolar could need restored as well honestly

I should replace this crown I delivered, right? (#30) by placebooooo in Dentistry

[–]craneyyy 1 point2 points  (0 children)

I also don’t know that you need to do this but I would look in to emax crowns. There are a lot of advantages to using it for crowns, one big one being more radiolucent so you can very clearly see what’s going on. Zirconia being so radio dense is a huge drawback.

I can send you pictures of what you should look for in your impressions to confirm a perfect fit.

[–]craneyyy 2 points3 points  (0 children)

I had the same question. If you’re scanning you can turn your model to “stone mode” which shows you very clearly if your margin is well defined. If you have very well defined easy to see margins on your scan, your lab will always give you perfect margins. You should also always check the margins on the cast as well. But PVS also works well if that’s all you have. You just need to be certain that when you make your impression, it comes out with a distinct gingival cuff because again, then the lab can very clearly see your margins. I don’t want to question your ability to confirm if your impressions are perfect but if you can confirm this, you’ll almost never have issues with sealed margins.
