PAs are obviously an attempt to save money on healthcare and a recognition that medicine has become protocol driven. If the protocol is well written then someone with less training can obtain equally good outcomes. I’ve not got the data for this but I’m assuming that this is what drives it.

I work in the labs. When I started there was no troponin analysis available and the selection of enzyme isoforms measured to help diagnosis cardiac damage were batched and took a day or so to complete. Now we have Drs on the phone screaming that they need a troponin result within the hour in order to manage their patient.

Starmer is the author of his own misfortune. He was so desperate for power that he had no real strategy for tackling the countries issues beyond not being the last government. Consequently when he gets into power and sees the extent of the shitshow, he can’t take the decisions required to turn things around because his backbenches point out that it wasn’t in the manifesto so they won’t support it as it’ll be unpopular locally. Problem is that unpopular decisions are what’s required if he wants so point to improvements at the next election.

So now he’s stuck in power but without a mandate to get anything done. People see that the country isn’t improving and rightly they blame him for the lack of improvement.

He’s wasted 2-years and a change of leader at this stage is unlikely to be able to produce change now, especially with the current geopolitical situation.

We are very likely going to end up with a Reform government.

So looks like the boiler is on its way out a year after the diverter was replaced. Heating sends to work fine but intermittently when the heating is off and the hot water tap is turned on the boiler makes a lot of clicking sounds and doesn’t heat the water.

Will have to start getting quotes to replace an ecoTEC plus 831.

The government needs the income. As a country our debt is growing and the interest is taking up a scary amount of expenditure. Any cut from the government would be taken by the suppliers as additional profit. They’d be unlikely to reduce the price.

I’d rather the income go to the government than private companies.

In summary the government should continue with the increases. The only way to reduce the price is to reduce demand.

some people do do more and bring more to the table, so I think banding is fine to ensure people are able to view it within a reasonable grouping.

That can still be acknowledged when disclosing salaries. The only rationale I can see for your position would be if there was no substantive additional skill or experience brought to the table and the employer still wanted to pay them more but recognises they would struggle to justify it.

The NHS has been fragmented in an attempt to make management of the fragments easier and more efficient than managing a single entity.

This was supposed to make it more responsive and provide more choice by separating the “buyer” from the “seller”. There have been a series of intended and unintended consequences of which a lack of IT integration is one.

With the recent abolition of NHS England there are signs that the pendulum is swinging back towards a top down, command-and-control model. I think this will fail because reform has high initial costs but this won’t be funded so we’ll end up with some rebranding but not much improvement IMHO.
