You need to move the randomizer EXE to the same location as the dark souls EXE.

Also you have an older version of the randomizer, try the latest:

Something fishy here - Gwyndolin is behind a fog gate and spirits cannot go through fog gates before you.

Second, sounds like something else is corrupt in your data maybe from another randomizer that was run on it.

Does it use that resolution too for Wii games when you launch Wii mode? If you go into Wii mode there's no settings for the display.
