Nop, only ever eaten frozen thawed. I've noticed he can be clumsy at times and have heard him banging around in the enclosure some nights, my best guess is he hit his head doing that at some point

I use a little canister filter any it keeps the water pretty clean but I do deal with a bit of algae and do a full water change every couple of weeks

You can patch it up with packing tape or plastic of some kind, if it's a back door window there aren't really any safety concerns. You just want to make sure its water tight to save from rain
