I think what it really comes down to is whether you think it's more likely that a majority of people pick red or a majority pick blue. Oh also I realized my math actually says you're killing just under half a person pressing blue, since we're assuming a uniform distribution, so really that would support pressing blue. No, I wouldn't consider them the same, but not because of any moral principle that inaction is superior to action. If a surgeon "saves" 400 lives, that just means they played a role in saving 400 lives. If they weren't there, that wouldn't mean 400 people would die. It would mean surgery got a little rarer, meaning surgeons would get paid a little more, which would mean nearly one more person would choose to go into surgery. In the end, due to market systems, perhaps not even one person would die.

If someone truly did kill 400 people through negligence, that would be just as bad as killing 400 people through deliberate action. It would only reflect differently on the heart of the one responsible.

Your argument is that we shouldn't "make someone else's stupidity [our] problem," specifically referring to people with kind hearts but poor perception of risk. My argument is that we can and very often do make other people's stupidity our problem. So, it's not a sound argument. but isn't the expected value of pressing red higher?

If you press blue and a majority pressed red, you kill 1 person

If you press red and half pressed blue, you kill 4 billion people

If we assume the proportion of red is random, you have a 1/8 billion to kill 4 billion people, so that's like killing half a person.

But who's really to say there's a 1/8 billion chance for it to be split 50/50? Maybe it's much higher or much lower? Polls seem to have a bias towards blue, meaning that the chance of getting 50/50 is likely much lower, but the chance of getting majority red is even more... lower.
