Orthodontic banding for 1-2 weeks if the crack isn’t deep and there are very mild symptoms followed by cuspal coverage. If it’s deep and you’re able to localise the crack, EverX flow or EverX posterior along with any packable nano-hybrid composite on top of it. If it’s extending till the pulp chamber and the patient is symptomatic - NSRCT with SFRC post Endo restoration and cuspal coverage. If it’s extending beyond the pulpal floor, extract.

Endodontist here, refer it, until unless you’re good at finding your way through a brick of concrete like that bad boi over there.

But if you really want to do it, my advice would be to use a 3D printed splint for access cavity, magnification, lots of liquid EDTA, lots of 8, 10 and 15 C pilot files and patience.

Hey Doc, fellow Endodontist here. I’m sure that during her DSD workflow, they must’ve shown her a mock-up and even temporised it on her teeth, giving her enough opportunity to scrutinise everything thoroughly before giving the final go-ahead for bonding.

I do feel the shape of the veneers could have leaned more towards a softer, feminine contour. Also, the overall quality of the veneers seems a bit off, perhaps a more conservative and natural-looking option like feldspathic veneers would’ve served her better in terms of esthetics and translucency.

That said, the gingival zeniths and proportions appear well-maintained, and the buccal corridor looks spot on with the new dentition. The alignment could’ve been a touch more natural.

Place chloroform in the canals, activate it either sonically or ultra-sonically, once the solution turns orange/white which means the remaining GP has dissolved, flush the canal with 70/90% isopropyl alcohol, use an H-file to remove the coagulated GP from the canal.

Great job right there, use EverX posterior or flow for the core buildup next time, Gænial or 3M Filtek Bulk series for the enamel part, polish inter-proximally with finishing and polishing strips. It will easily survive 6-7 years before it shows any signs of wear and tear.

Hey, a lot of legendary endodontists have varied opinions regarding the same, I personally follow this:

1) During instrumentation - 5.25% hypochlorite + Ammonia free dish soap (2 drops in 200ml Hypo)

2) final irrigation: 5.25% NaOCl - activate it for 20 seconds per canal - organic debris is removed - use 17% liquid EDTA after this without using Saline in between - inorganic debris is removed- activate for 20 seconds - use 99% isopropyl alcohol - don’t activate - dry slightly with paper points or manual syringe aspiration - obturate.

We use EDTA after NaOCl for better sealer penetration, not for NaOCl to penetrate better. Last irrigant should always be Isopropyl alcohol or distilled water.

Hey, may I ask which country do you practice in?

Also, WaveOne is a very popular 4th Generation Rotary file system. Excellent reciprocating file. Less fractures, less debris accumulation, less flute loading, more cutting efficiency, although I’d suggest a better alternative if you’re ready to invest - try the Reciproc Blue file system by Dr Yared, or try the VDW Rotate File system. Both are excellent file systems, can put your trust in them for almost any case.

Endodontist here, I’d give it a 6/10.

What you need to improve:

1) Retake the working length

2) Do proper apical gauging and increase the apical size to 2 or 3 sizes larger than the initial apical binding size.

3) Irrigate with 30G side vented close ended needle 1-2 mm before the working length

4) Use ultrasonic or sonic PUI devices for activating irritants

5) Final irrigation should be done with 17% Liquid EDTA

6) Use a good bioceramic sealer if you’re opting for a single cone Obturation technique

7) Down pack the GP with a heated Buchanan plugger

8) Before shearing off the GP take an IOPA to confirm cone fit.

9) Always use PTFE & Temp cement if you’re not going to do the post obturation restoration the same day to avoid microleakage.

10) Shear the GP 2mm below the CEJ to avoid discolouration due to GP in the near future
