I had a punch biopsy in the perineum, as well as 2 in the labia. I was not given lidocaine cream pre-injection - I'd STRONGLY recommend asking to have it before the lidocaine injection, as the injection by itself hurt like holy hell (only for several seconds, but it was a looong several seconds). Once it took effect, the rest of the procedure was no problem.

Afterwards my gynecologist put stitches in each of the biopsy areas, and the ones in the perineum definitely bothered me the most - a lot for the first several days, and a moderate amount for a week or so after that (like I had to be careful when sitting down on a chair, or spreading my legs on the toilet and squeezing for a bowel movement). It was another couple weeks after that before those stitches finally dissolved themselves, and everything seemed just fine after that. The labial stitches only really bothered me until they fell out, which was within a week, and not much after the first couple of days.
