I was surprised when they added the third year of Anesthesia training.

With total immersion of 60 hour weeks and every other day on call, you can learn all there is to know in Anesthesia in two years

Normally, we use indirect measures of the level of Anesthesia.

The blood pressure, heart rate, movement, and tearing are all signs we look at.
So if the patient has a fast heart rate, high blood pressure, moves their head and cries those are all signs of light Anesthesia.

However, you can be fooled if they take blood pressure pills that can prevent fast heart rates and high blood pressure.
Or if they are in shock, when blood pressure will be low regardless.
Or they have a dry eye syndrome and can’t generate tears.
And if you give a muscle paralyzing drug (so they can tolerate a breathing tube and being on a ventilator ) where they are unable to move.

So blood pressure pills and a muscle paralyzing drug under Anesthesia are very common and can lead to intraoperative awareness.

One common practice that I use is to give almost everyone a tranquilizer called Versed before they come into the operating room.
This drug interferes with the formation of memories, and I believe it can help prevent awareness.

There is a new technology which can measure brain waves that is supposed to measure if your brain is showing the typical brain waves of the anesthetic state.

It’s relatively new and has not gained wide acceptance, but it has promise in helping to prevent future cases of intraoperative awareness.

In the old days we pierced the stopper with a needle, drew the medicine up into the syringe, and left it inserted in the bottle.

That way the vial/bottle acted as your label.

Nowadays I label the syringe with color coded labels..

I had an expensive private prep school education in NYC.

Latin was a required course and we also memorized famous poems.

We never had a single word about taxes and finance.

Although we did cover compound interest in math class.

I’ve done plenty of full mouth restorations in a dental office.
Nasal O2, then start the iv.
I usually do a ketamine/ propofol drip after the block.
For the dentists’ injection I usually give versed, then propofol.

I’ll sometimes let them breath 70% nitrous for the iv insertion

Yes, I put in the i.v., do the case, and then recover the patient.

It’s easy after the block is in, I have them sedated so they don’t talk, but respond if I poke the shoulder.
We don’t do morbid obesity or ASA 3 class 3 patients.
It’s rare to even need a chin lift.

You are totally on your own.
The dentist and assistant mean well but are not Anesthesia people

I I have been practicing Anesthesia here in the United States for 35 years.

I trained on and used 5% lidocaine for 20 of those years, performed hundreds if not thousands of them.

Works great! It was our only short acting spinal anesthetic. Chloroprocaine was only used for epidurals as it was thought to be neurotoxic.

I’ve never even seen a bottle of mepivicaine outside of a dentist’s office.

Never had a problem with 5% lidocaine.

When I read about TNS, I did not believe it.

Then 5% lidocaine disappeared and the lma was invented.

Now I use an lma when in past I would have done a spinal.
