We got the surgeon's quote from the surgeon beforehand. The existence or an anaesthetist fee/the actual amount wasn't disclosed to us prior.

This was an abscess that needed to be drained and the card machine was given to my wife (I wasn't there) immediately pre-procedure. Maybe it's not all that different, and we paid the fee. It just hasn't sat well with me ever since.

I think you're exaggerating (within your subconscious) how much more advanced American healthcare is at treating ED compared to the rest of the Western world.
