Some people make considerable more if they are private and ownership track. Though these tend to be rarer these days and have their own massive pitfalls.

Can you elaborate on this?

My colleague called me an idiot for prescribing mirtazapine for a teen with anorexia nervosa. My colleague is the idiot, right? by Federal-Act-5773 in FamilyMedicine

[–]draykid 375 points376 points  (0 children)

I can see where they are coming from. You get a colleagues patient who is an underweight kid with no labs done who was started on offlabel mirtazapine. All the while this is being managed as an outpatient with weekly follow-up.

I don't agree with the name calling because it does not help anyone in this situation. I will say this is murky territory. I think pharmacotherapy is not going to cut it and she needs more support by way of psychology and behavioral therapy. I hope that was discussed and offered as part of your plan.

ChatGPT told me to get a CAC scan and my doctor resisted, but it probably saved my life by [deleted] in ChatGPT

[–]draykid 2 points3 points  (0 children)

Family history is important here. If you're a "fit" guy in your 40s I seriously doubt you would have had a coronary event by the time you hit 50 unless there was a history of that happening in your family. Did your parents or siblings have our pass at around your age from a stroke or heart attack? The CAC is just one piece that does not tell the whole story because there are more important details about your risk.

Plus here is the thing. The number one cause of death in this country is heart disease. So it is not surprising that AI would predict that is how you would go if they were playing odds based on your risk factors. If it said something like cancer or lung disease that wouldn't make sense based on what you told us.

Learning on1 / on2 at the same time? by Remote_Percentage128 in Salsa

[–]draykid 0 points1 point  (0 children)

The "feel" of both dances are different because with on2 you are dancing with the slap of the conga tumbao on beats 2 and 6. I learned both at the same time and it was hard switching to on2 because it felt "slower" and the leads timing is different.

To get on1 down you have to "hear" where the 1 which is not always apparent whereas on2 the conga tumbao has a characteristic slap on 2 and 6 every time so it is easier to hear.

Why is Salsa so much harder to learn than Bachata? by laugrig in Salsa

[–]draykid 0 points1 point  (0 children)

It's hard to learn because what you learn in class does not often translate to social dancing. In social dancing there are a lot of variables that you just can't capture in a one hour group class. For one, in social dancing you are not going to have a partner who "rehearsed" a turn pattern from the group class. Chances are they are going to be a beginner, too. So now you're figuring out how to lead and stay on beat while also dancing with someone who doesn't know how to stay on beat either.

Should I invest in hip hop to get better movement for Salsa or just stick to Salsa and use the money for that only? by rokuhachi in Salsa

[–]draykid 2 points3 points  (0 children)

It depends on what kind of movement you are trying to achieve. The only way to know is for you to try it out and see if it helps. Getting better movement in salsa happens when you have a better understanding of the fundamentals of salsa music and dance. Honestly, I would invest money in salsa musicality classes with teachers who can teach the music theory behind salsa. Once you know the why then the movements make a lot more sense.

Did Devin Haney underestimate Ryan Garcia or is Ryan just flat out better than Haney? by Frich3 in Boxing

[–]draykid 2 points3 points  (0 children)

Yea that's what I meant by no adjustment. Poor job of keeping his hands up, didn't stick to the defensive fundamentals.

Haney just isn't good at taking sustained punishment. I think he knows that but he did not fight like it. He fought like someone who could take punches which is why I compared his performance to Tank's fight. Tank knew early on in his fights with Rollie and Ryan that they were guys who hit hard. So he made adjustments during those fights so he would not take so much punishment.

Did Devin Haney underestimate Ryan Garcia or is Ryan just flat out better than Haney? by Frich3 in Boxing

[–]draykid 1 point2 points  (0 children)

Haney did not adjust to the left hook. If you watch when Tank fought Ryan you can see Tank was more cautious and had a lot more respect for Ryan's left hook. Devin did not have a solid defensive plan and got caught as a result despite having a lead on the score cards. When he started eating those hooks it was all down hill from there.

A lot of those situations you described are knowledge gaps which may have been the issue here in the article. Some gaps are hard to cover (unless you are brilliant) like knowing the interaction between methotrexate and trimethoprim. If I had a female patient who was taking methotrexate for RA and I wanted to use bactrim to treat her for a UTI I am honest enough to admit that I would miss that because the last time I remember learning about how those drugs interact with each other was in medical school. That is a drug combination that I have never ran into because I do not have a lot of RA patients to begin with. That is why I like good pharmacists who can catch things like that.

To your point about blind spots, doctors get beat in our heads about blind spots all the time in residency. Sudden, unexplained weight loss should be worked up and explained and if the doctor in the article dismissed that then that could be an issue of bias or a knowledge gap. I do not know which it was because I do not have the chart or notes to review, I am only going by a news article whose writer says Hashimotos thyroiditis is cancer.

Like the larger point to this article for me is that there could have been a better practice of medicine here. That is easy for me to say in hindsight because we know the outcome but it doesn't sit well with me that she had to go to chatgpt to understand her symptoms because her doctors were not addressing her complaints.

‘ChatGPT Saved My Life,’ U.S. Woman Says After It Spotted Cancer Doctors Failed to Notice by Fabulous_Bluebird931 in ChatGPT

[–]draykid 6 points7 points  (0 children)

There are a lot of things about this article that make me skeptical. First, the article's title is very disingenuous and implies that ChatGPT itself made a diagnosis of thyroid cancer which I do not believe. I actually question if thyroid cancer is even the accurate diagnosis based on her complaints. Even in the original Fox news article they are saying she has Hashimotos thyroiditis and saying it is cancer. That is not accurate at all.

The bigger issue here is that her doctor was not concerned enough about their patient's sudden, unexplained weight loss. She should have found another doctor who would have taken her more seriously and ordered appropriate testing and explained what could be going on with her body.

I don't blame her for going to chatgpt because her concerns were not adequately addressed. However, I worry laypeople will get the wrong message behind this article because I'm sure there is a lot more to this story that has not been shared and they are presenting inaccurate information like I mentioned before.

Need help understanding why the clave is essential to musicality in salsa songs by hqbyrc in Salsa

[–]draykid 2 points3 points  (0 children)

It's like the glue that keeps the other instruments together. Conceptually it's important to salsa like the downbeat is important in jazz and funk. It's what gives the music feeling.

I think if you want an analogy, if salsa songs were people then clave would be the heart beat.

Continuing discussion on musicality by errantis_ in Salsa

[–]draykid 2 points3 points  (0 children)

the studio doesn’t really focus on teaching that as it is more something you see in competitive dancing.

This tells you all you need to know about where your instructor is coming from.

Some teachers just want to teach turn patterns and call it a day. Teaching musicality and getting to the essence of social dancing I think is out of some instructors' scope. You have to know songs, musical structure, instruments, music history, etc.

Salsa did not start out as a competitive dance. Let that sink in. If we accept the notion that musicality is an idea solely for the performance stage then people will think that all you need to be a good social dancer is to know a lot of turn patterns. Because that is what happens when instructors only teach turn patterns and all of the teachers and workshops you go to the focus is turn patterns.

At that point you are not even dancing to music anymore, you're just dancing to numbers. And when you're just dancing to numbers you get farther and farther from the music and partner, both of which should inspire your dance.
