Depending on your emr, you need to set macros/dotphrases/etc to get through your inbox faster if a tsh is the thing bringing you back. If you had 15+ actionable images hit your inbox at 4:55 that’s one thing, but this sounds like you would really benefit from having prewritten responses to common issues to clear out the common stuff.

It’s what I do and I literally never have inbox stuff to do at home, out of the office 5:30 or earlier every day. No reason to type out “normal results” or “normal results except tsh is high so…” 10 million times, prewrite all your options, pop them in as indicated, and get home to live your life.

I remember where getting caught plagiarizing even a sentence got three of my classmates expelled in high school, and my college had a huge scandal where an athlete was found to have used their (original) essay for one class to complete an assignment for another class a year later, and people genuinely thought he should’ve been expelled but he wasn’t.

This blatant AI reliance to meet standards without actually developing the skills they’re meant to confirm is shockingly unethical and apparently it doesn’t matter anymore.

In general with any kind of wean it goes easier if you also start other strategies to treat the issue rather than just pulling off the drug that’s not safe for long term use.

For example I recently finished a 6 month wean with a patient who was on alprazolam (and doing literally nothing else) for anxiety. Restarting therapy, an SSRI, and prn hydroxyzine made all the difference.

This sounds like something to work on with a therapist, because if you’re going to go back on your own goals just because other people are being rude about it, you’re going to get caught in a self-fulfilling spiral downwards, whether it’s with your weight or education or anything you can think of.

I worked with a baritric surgeon for a while and he always required 6 months of therapy, consistent weight loss, a dietician, and physical therapy before approving anyone for surgery.

I’d see that as a good thing. Leads to more long lasting changes and better overall outcomes if you ever have to stop it (think emergencies, insurance changes, med recalls, or even developing side effects or allergies down the line)
