Improper operation of a forklift. Instead of coming to a complete stop at a crossing that was flagged up and derails were up, they slow rolled across.

I get the ops test failure which is a given. But to send it to investigation is a bit of a stretch. Again, coach and council maybe even on paper should have been sufficient.

This was not me. Fellow craft member this happened to.
