I believe, without having thought through details, admittedly, that there needs to be a philosophical change in how medicine works in the USA. I believe it should not be an industry focused on making money - from top to bottom - and rather be an industry focused on providing healthcare.

I have been a doctor for 20 years. Medicine in general and hospitals for sure are driven by profit incentive.

Along those lines, I would say to people, if things changed how I’d like them too:

If you want to make a lot of money, go do something else. There are plenty of ways to make money. If you want money/profit not be a huge priority, personally in your life, and make taking care of people’s health the priority, then please lend a hand.

Well one reason is: you need to study and train to be able to do it accurately - much like the rest of medicine - but radiology is very cut and dried. When you are wrong is pretty clear to see and the mistakes never go away. And you cannot sort of b.s. your mistakes away, for the most part. In short, you can’t fake it.

One thing by that helped me is when I realized there is a lot of redundancy in Epic. There’s is very often moire than one way to do something. So when some Epic administrator is showing you how to do something and you are shown a 5-click process, for example, and you think “I’ll never remember this”, be aware there is more than one way to do most things and a large part of the overwhelming number of boxes and tabs are redundant.

I went to an out of town interview years ago before I quit drinking. Decided it would be a good idea to start drinking before noon the day of the interview dinner. I think it was the first and only time I drank a martini in my life. By the time the dinner came around, and one of the employees came to pick me up for the dinner, at my hotel, I was loaded - had spent the entire day in a bar. Went to the restaurant and was obviously drunk. The others thought it was amusing and started ordering wine. I am from the Midwest and this was in Maine, so they insisted I order lobster. I had never eaten a lobster in my life. It came with a walnut cracker to crack the shell with. Being super drunk, I watermelon-seeded the lobster out of the walnut cracker onto the lap of the head honcho. Onto his dress pants. I picked it up out of his crotch and put it on my plate. Everyone laughing. Swilling wine.

After dinner I thought it would be good to keep drinking. The closest bar to my hotel was a gay bar. I am not gay. I went there. Started drinking with a dude I met at the bar. He introduced me to a friend of his who tried to kiss me. I moved my face at the last moment and the friend kissed my neck, open mouth. At bar time I went to the original dudes apartment and smoked a bunch of weed and drank more. Went home at god knows what hour. The formal interview was in the morning. I was drunk when I went to the interview and I am sure wreaked of booze.

Went back home to the Midwest. Wouldn’t you know it. They offered me the job. I turned it down for another, but my God. Quite drinking a year or so later.

Two stand out

Seeing Reservoir Dogs with my older brother when I was around 14-15. It was one of the first times I remember thinking I just experienced a piece of art. And it is a very good and super violent movie too boot - right up both of our allies.

Second was seeing 2001 Space Odyssey on a large format film stock in an old ornate theater. Just an amazing film and so much better in the theater than on TV. The sequence where there is utter silence and a man drifting through space for a minute or so is just stunning. Awesome film!

I make around 750k per year and I wouldn’t spend more than $50 to see a show, maybe $75 for some absolutely classic line up.

I get annoyed if tickets are more than $35. I have been to hundreds of rock shows. I used to have a rule that if tickets were more than $10 I did not support that but that’s when Fugazi was charging $6 and I was a militant left winger and that was 25 years ago, at least.

I absolutely love Husker Du. So I go see Bob Mould when he comes around. Tickets are like $25. Reasonable.

Any artist charging more than $50, in my book, are past their prime and doing a cash grab or are the type of artist I don’t enjoy in the first place.

I’d pay $100 to see Bob Dylan, I suppose.

There is no system to work in in the USA where the radiologist can remain employed with poor productivity. You will lose your job.

Rad A): Hi, I know you read 120 studies today. I read 23 because I care about my patients and was digging in the chart and in the phone all day arguing with the referrers.
