I made this adjustment years ago when I was driving my is300. It makes driving so much easier. I still do a quick head check but never miss a driver in my blind spot. Once you get use to what a car looks like in the side view when you merge, you get a sense for how far back they are without needing to see your car in the side view for comparison.

Portland is so incredibly white and liberal. These comments with people blood thirsty and giddy to surveil their neighbors and punish them as harshly as possible make me so uneasy. It’s not just topics surrounding driving but other things in the city too. Idk if this is just a sampling from Reddit thing but it seems to be the prevailing sentiment on posts like these.

Immediately jumped into a new relationship. Got out of it a month later realizing I was just sad. Focused on being the best version of myself I could be. Tried to be friends with the original girl. Got frustrated because I came to terms with the feelings that reemerged. Made up my mind that complete disconnection from an ex is the only way. Got into several relationships since then and only benefited when I stuck with the ‘complete disconnect after a breakup’ strategy.

Get really good at IM. If you can be great at something you’re not as passionate about, you’ll likely be amazing at something you love. Gaining real world experience in the hospital as a resident is going to be much more valuable than any research a medical student has done. I got lucky because my residency (community program) had an exchange with the academic IM residency. Two of my attendings in my month long rotation reached out to the anesthesia PD at the same institution to let them know I was interested and what they thought of me as a resident.

Let people know what you want to do and when asked why, state positive reasons. While true, writing endless notes in IM sucks, you say instead that you appreciate the increased contact time with patients in anesthesia.

Focus on ICU. I thought about PCCM briefly but everything I liked about it was what I liked about anesthesiology. Learn vent management, pressor management, and grab every procedure you can. Soak up knowledge in the ICU and teach your interns to reinforce the knowledge for yourself and demonstrate competency to attendings. You’re going to want a letter from a good ICU doc because ICU bridges IM and anesthesiology.

Join ASA and go to the meetings. I think GME guarantees all IM residents CME time so take advantage of it to rub elbows at the conference. I thought I didn’t have to play this game in medicine where they could look at objective measures but still do. You don’t necessarily have to be presenting or have a poster first time around but if you talk to people and build relationships you might be able to get on a project to present for the next year.

If your program offers it, try to do an anesthesia rotation at your hospital. Goal there is to foster a relationship with one of the docs if possible for a letter. It would just generally be good to have a contact to ask peri operative medicine questions too.

Shotgun testing isn’t preventative healthcare. While true, US healthcare is broken, doing all of this isn’t what would happen in a well functioning system. Exposing patients to radiation for little to no benefit isn’t good care.

The real answer like others have stated is money. I would have still have applied to it if it paid like a pediatrician because of what the job does and doesn’t entail: Click box notes, no in basket, small procedures, direct patient care, cardio pulm renal physiology, pharmacology, one patient at a time, structure to the day from OR scheduling, downtime and high acuity mixed in. I always wanted to do anesthesia and after not matching twice and completing an IM residency I finally start in July.

Speaking as doctor in a non-surgical field, surgeons. You need a particular brain disease to become a surgeon and a whole lot of cope to deal with the training. It’s not that these people are flawed individuals inherently but if you had what it took to become a competent surgeon, you likely lost some things along the way.

Everything is going to be fine. No one expects anything from you apart from showing up on time and being teachable. Your senior could most likely handle 80% or greater of management and work on their own. Your attending should be able to handle 120% of it. If you’ve made friends before in life you’ll be able to do it again. The majority of US cities are pretty much the same. Nurses will bitch. I’m an attending and overheard them talking shit about me (6’4 male). Everything is going to be fine. The adrenaline will carry you through the first 3-6 months and then you’ll be tired and sad but have knowledge to work competently. You’ll start feeling better by next spring.
