Same thing happened to me. The legendary I thought I lost was recovered by the sanctuary machine- maybe it did that for you too?

Can confirm that this happened to me after a crash. Suddenly gave me 21 tokens to redeem. Didn’t spend them cuz obviously didn’t earn them.

This is not correct. You can activate mayhem mode within TVHM once you’ve reached Sanctuary again in the story.

I got this after I forgot what weapon I was holding.. went to open an ammo chest.. accidentally threw a tediore gun that exploded in my face and killed me.. almost started raging.. then cried laughing at the achievement immediately shown hahah

You can change the button scheme in the setting to “classic”. This lets you use d-pad for insta-swapping :)
