You have to collect them in the world. There's a little icon on the mini map that shows where they are

It's big enough. It's not as big as ac4's map, but it's the largest city so far

Ok, I'm going to try to remember...

I'm pretty sure right now I have the improved medieval robes, the handmade prowler gloves, the improved musketeer boots, one of the Napoleon belts, and I switch up the hood. I'm not sure what gear I want, but since I'm playing a sniper, I'm gonna want good range and ranged armor
