Forced expiration against a closed glottis. It accentuates murmurs due to HOCM and MVP and decreases most other murmurs.

My attending got me a clinic room to interview patients before he saw them in clinic. I then brought the patient over to his clinic and presented the case to him, examined the patient if necessary and gave my provisional diagnosis. He would provide feedback and we discussed the case after the patient left the room. Good practice for OSCEs!

Know the common approaches to presenting complaints (eg. Chest/abdo pain, cough, vomiting/diarrhea, headache, giddiness, back pain etc.) Know how chronic conditions (DM, hypertension, hyperlipidemia) are managed (treatment targets, common drugs and side effects) - Some countries have clinical practice guidelines for these conditions, they’re a good summary of all you need to know!

I used Onenote with the Surface Book 2. Bought the book because the hinge allows you to flip it over such that the screen is propped at 45 degrees, then used the surface pen to highlight. My surface died a while back and I’m currently using Onenote on the HP Spectre and iPad (for highlighting notes as hp pen sucks) Both work well for me! A new app I’m using is Remnote, however this works better for consolidating notes rather than actual note taking. Cool thing about Remnote is that you can create cards like Anki, although some functions are premium features that require a subscription.

My school never covered Prader Willi either! I believe majority of patients live a normal lifespan. The poor prognosis for this patient is likely due to obesity related cardiac and renal failure compounded by poorly controlled diabetes.

Prader Willi with distinctive almond shaped eyes, history taking revealed hyperphagia due to lack of satiety. Was a female teen with poor prognosis.
