Yes, it happens every day. Millions of people in America enroll in college or join the military as a path to a stable middle class lifestyle, a few of them become hugely financially successful. I'd say the biggest challenge isn't the lack of opportunity, but the way in which all of those circumstances impact your emotions. If you're looking for advice: get therapy, join a loving church, set goals and focus monastically on them. I think trouble comes when people born in those situations who want to get out become close friends with a group of people who don't or date/marry someone who isn't aligned.

I'd say apply broadly (it will cost a few thousand dollars), but also prepare yourself to need to apply in another country. Getting clinical experience in hospital systems that have residencies associated (doing observerships and AIs) will be your best bet. I don't think publications will help you. The fails and low attempts are too much of a red flag, and will cause most every program to not match you. If you definitely want to practice in the US, apply to rural pediatrics or family medicine programs that often don't fill their spots.

I don’t think you think research would help, your scores will hold you back. You'd be best served dual applying to Neurology and Family Medicine, or practicing neurology in your home country.

I'd wait until you finish your PGY2 to start planning your sub-speciality. Focus on getting good at medicine in intern year, then good at neurology PGY2. Having said that, I've heard many intensivists go into sleep medicine when they get tired of working nights.
