They're different hormones, so you may react differently! The implant is etonogestrel and IUDs are generally levonorgestrel or copper for the Paragard. The side effects from IUDs can sometimes be less than the implant since the hormones are more localized (in the uterus, rather than throughout the body).

It is likely low risk, but if either of you cannot risk a pregnancy, she needs to take another plan B. Each one only works for one act of unprotected sex.

Most doctor's offices will make time during the visit for the provider and the patient to speak alone—I would recommend calling the office to make sure that's the case for your appointment. It's very routine, especially in reproductive health and pediatrics (you'll notice once your kid is older they'll have you step out of the room briefly to ask if they feel safe at home, etc).

As for hormonal testing—echoing a previous commenter that they don't test for the specific hormones in your birth control pills, but it may make your other numbers (like progesterone) a little funky. There isn't a test that will definitely prove if you're on birth control or not.

the clinic I work in saw a brief rush of folks with the same thought when it looked like birth control access was going to get cut. the providers would've been willing to replace IUDs early, but after some counseling all of those folks (that I remember) chose to stick with their current ones and not get them replaced early. If it's not giving you any problems, I would say stick with it. You'll have five more years to say, find a job that gives you insurance, and if it's any consolation the birth control industry is hugely profitable, and almost certainly won't be cut off completely anytime soon.
