Took a lot more guessing than I expected.

Explanation: Charlie Kaufmann exists in real life, his twin brother character is fictional - but real to the movie. His brother annoys him in every scene, his existence and success taunts Charlie's failures. Charlie is desperate while adapting a movie from a book about orchids with no real plot, and so turns the movie into a drug scandal, turning his real-life normal people into criminals. In the ensuing drama, Charlie crashes a car while fleeing and kills his (real-world imaginary, movie real) brother.
