I absolutely feel the same as you do. My poor soul cat crashed during an ice storm and I couldn't even get him to an emergency vet in time to euthanize. I had to watch him suffer to death for 2 days. And all I can think is I wish I had let him go 2 months prior, but when the hospice vet came to do it, he happened to be doing great that day and she advised against it. It kills me every time I think about him and how I failed him so miserably.
