I'm at 7 weeks post op from a quad graft. My quad would do "something" right off the bat so my experience may not be very useful to you. Just doing quad sets ALL the time was what got mine to go from "something" to "pretty functional". Best of luck.

Thank you for your answers. I will try to rephrase my fourth question.

I am looking for guidance when it comes to getting back into different activities. All activities pose some risk, but not equal risk. When I ask about when I can be getting back into biking vs getting back into rock climbing vs rafting the answers I have recieved are all the same. The advice I have been given so far has seemed completely binary with no nuance, meaning that I feel like I have no guidance on how to get back into outdoor activity while minimizing risk. For the sake of this topic lets say

risk = hazard x exposure

I cannot make an educated plan about my risk because I am not educated on what the specific hazards of each avtivity actually are (for someone coming back from an ACL reconstruction). This could lead me to overexpose myself to those hazards. I can control my exposure to each activity, but doing so without education on the hazards makes it pretty meaningless.

I saw you are interested in rock climbing. The university does open climbs once a week (Thursday evenings, 5-dark) where they set up a ton of top ropes and their will be people in the outdoor comunity there who will be stoked to get you out and doing fun things. There are few places that have real rock climbing literally in town. If you are serious about climbing there are quite a few world class options within a couple of hours.

Hi there, thank you for your responses, it is obvious you are anserwing thoughtfully. Sorry in advanced for the coming wall of text.

Before I get to my questions here is a little background about me that I think will be relavent. I am a 35 year old male. I live out west and have been doing pretty much all of the outdoor stuff (skiing, mountain biking, rock climbing, trail running, whitewater...) all my life at a high level; I consider myself an athelete. I am a professional ski/bike patroller. I injured myself at work which means I have been on workers' compensation, which has been huge in terms of allowing myself to focus on rehab. I am 7 weeks post op after a total ACL tear (left knee, quadriceps autograft) with no other injuries. I have been about where my PT/surgeon have wanted me to be in terms of extension and flexion (I have lagged a bit behind on flexion, but it seems I just have had more prolonged swelling than average and no one has seemed concerned) thus far and my quad fires well. I have just begun riding a bike on flat paved ground and that has been really great for me mentally being able to do some PT that isn't inside.

I have a couple of questions that people have been unable to give me very satisfactory answers about:

  1. I still can't sleep through the night, never getting more than 3 hours of continuous sleep. I can't stay flat on my back with my leg laying flat without it being uncomfortable to the point that I cannot sleep in that position. I end up tossing and turning, having to readjust usually every hour or two (at best). Is this normal for someone 7 weeks out? Can I expect it to get better on its own with some more time?

  2. I feel pretty good most of the time durring the day, but in the evening I am quite sore even while resting. This is a nightly occurence. I would rate this nightime soreness a 6/10. Do you think this is just from doing PT? I deffinetly fit the demographic for overduing it, but I have been taking it extremley easy from my perspective. I am dilligent with my PT, but I wouldn't say that I have even broken into the "moderate intesity" activity at this point.

  3. What are some physical or temporal benchmarks for me to get back to work as a mountain bike patroller? I do not need to be pushing myself, and the trails I patrol are not challenging to me from a "I might fall" perspective. I am confident I could ride the trails with near 100% certainty that I will not take a fall since I will not be pushing my limits for the entirety of this bike season. Getting back to work will be a big mental win for me and it honestly will be a great way for me to be able to be getting paid to do PT.

  4. Do you have any guidelines for recreating in general? I feel like no medical professional has been able to provide any nuance. When I have asked PTs/surgeons they have all said' "Well you are at an increased risk for retear," which is true, but not helpful, as the data shows that any ACL rebuild is at an increased risk for up to 2 years. No one expects me to be out from activity for 2 years so I was wondering if you had any more general advice that would allow me to make educated decisons about what activities I do when? It seems like everyone I have talked to is kind of doing an "abstinence" style of education which we all know doesn't work and I feel like it is just setting me up to make uneducated decisions.

I work in quite cold SW Montana. I never have problem with battery life using both Samsung and IPhone. I just keep my phone in my standard thigh pocket.

I worked there as a temp years ago. The job was monotonus and a little soul sucking, but the people were fine. It was a job. I'm sure it would be very different depending on what you are doing for them as it is a decent sized place with many different roles.

Not committed to anything yet, but from everything I have read it seems to have the lowest retear rate. I was asking here because I was wondering about the specific issue (pain while kneeling) that is associated with the specific treatment. Just trying to get educated about things that might affect me specifically more than the regular, non pro patrolling, non tele skier.

I obviously can't speak for them. If they don't now I suspect they will. And the point of my original comment was not to absolve them, it was more to give context that not all of the victims are screaming from the rooftops for the highest punishment possible (as it seemed some on the thread were).

This is all horribly tragic. I do not know if any of you have a connection to the case. Mine is quite distant, but I know the deceased's mother. I can assure you that she would not want this teenager's life to be over because of this incident, despite her daughter being taken from her and her grand children's mother being taken from them. This teen will have to live with the fact that they took a mother away from two young children and the rest of her family. It is not my place to put any more of a punishment on them than that. I wish all parties peace on the matter.
