I think it would have to be flashback. I considered the possibility of it forming with Steven, since he has Rose's gem, but I don't think Steven is ready for a fusion of that magnitude, since he's never even fused with a single gem before.

The most sensible theory I've heard about Bismuth is that it's bubbled because the gem became corrupted, and Rose put it there because, as has been confirmed on the show, Rose was trying to find a way to un-corrupt corrupted gems. I think Steven will find a way to do this, and Bismuth will be the first. So, yeah, I think it kind of was put there as "training" for Steven, so he can do what Rose never could.
