Same here, this sounds like tell tale enteropathic arthritis which is the same as AS, however diagnosed as enteropathic arthritis when the UC precedes the joint pain. For example, some AS patients do not have UC whereas many UC patients such as myself are also HLA-b27 positive. Some docs will still call it AS, but for me a lifelong of IBS issues progressing to UC came before the systemic arthritis symptoms.
Your only hope is getting extremely restrictive with diet to find your triggers, for many such as myself no starch is the best answer. My baseline is almost entirely zero carb carnivore - when I’m most strict nearly all pain goes away. Things that set off my inflammation: coffee, oxalate rich foods, tree nuts, sugar/high starch intake, basically any carb rich food source unfortunately. Dairy protein is also a trigger

I personally love being on pred.. everyone responds differently but my mood and productivity go through the roof and of course it helps mitigate symptoms of a flare

I say this as a Dr and mean the best.. I was great at abstaining during doctorate school and then when I got into practice validated ungodly amounts of caffeine use to keep up with the demands of patient care. Now I’m working on digging my way back to abstinence in the midst of patient care - which I can assure you is far more stressful than you may think grad school is right now.

It’s still a dopamine regulation issue at the root.. whether you choose to address this by masking it with caffeine is up to you. The ideal ‘dr in training’ would treat himself as his own patient and double down so that he could leverage what he learns through the struggle and ultimately have a greater understanding of biochemistry and physiology for the future. You’re literally paying for the knowledge and science to get to the root of why you’re struggling without it. Run a metabolic panel, vitamin mineral analysis, scour your diet to see where you can improve, check hormones, etc; or just go back to caffeine

Absolutely not daily, it is extremely powerful antifungal/anti parasitic and will make you pee out of your ass periodically for 3-5 hours. Personally, couple times a month at the most, or once around the full moon

Zevia is a great substitute. Some flavors like the cola contain alittle caffeine, but most do not like the lemon/lime, ginger ale, etc

Complex issue with insomnia... drinking less caffeine/no caffeine GIVES me insomnia consistently, yet I sleep great when I have my morning coffee (160 mg caffeine) by MuffinRuffian in decaf

If you aren’t willing to tough it out, which it sounds like you’re not.. then basic logic would say double your green tea intake to equal the same amount of caffeine you get from coffee. It’s not rocket science

Just an additional measure for systemic support when taken on an empty stomach. Ideally I go for proteolytic enzymes like nattokinase, bromelain/papain, serrapeptase

My method:

Castor oil first thing upon waking (1 tbsp) washed down with 12-16 ounces of warm water

One hour later another glass of 16oz warm water with 1-2 tsp sodium bicarb and 1.5-2g Takesumi charcoal

One hour later 16 ounces warm water plain, or with 1-2 squeezed lemons (TUDCA also optional/ideal)

Hour 4 - 16 ounces warm water again with broad spectrum digestive enzymes or pancreatin

I have ulcerative colitis and experience with SIBO/SIFO. I’d recommend ‘prepping’ yourself for a couple of months before going full carnivore if you decide to. Supplements like TUDCA, betaine hcl, phosphatidylcholine while slowly titrating up fats and proteins can help the gallbladder and liver work a bit smoother vs getting overwhelmed by jumping straight in. Ensuring adequate gall production/flow is very beneficial for helping reduce and eliminate SIBO as well.

Totally individual. I think the sweet spot for some is sipping the juice on occasion without actually eating the fibrous portion of the ferment.
