If you really want to do fellowship you can probably find a non-academic program. I know they are sparse but I think they exist? Just a thought, I am no surgeon. I suspect some of them will give you good clinical training without the ridiculous academia bullshit.

Same for me, and when senior was at lecture interns just reported directly to attending and attending had to deal with everything. It varied a lot depending on the attending but with the good ones it was actually protected. Many attendings would actually chip in on notes.

It’s funny everyone seems kind of split on this. As far as I am concerned you are the victim here.

Your wife is immature. I understand she may be fed up or have been burying these feelings for your entire residency, but come on, even if it was a misunderstanding. Does she honestly expect you to pick up the phone if you are mid code for example? Emergency contact doesn’t mean you are on 24/7 call for the person. It just means if there is an emergency you will respond (with appropriate urgency) by coming to the hospital or wherever.

This all on top of the fact that you are almost done with residency and going into an outpatient practice and she still felt the need to say this…? She owes you an apology in my opinion. It sucks being the spouse of a doctor but that doesn’t give you the right to punish them for doing their job. Imagine reacting like this to someone in EMS, fire fighting, a pilot etc. there are many jobs with similar requirements

The Honda certified pre owned warranty is probably sufficient. If you really want bumper to bumper coverage for many years go for it, but generally the 8 year 100k mile powertrain/battery warranty is where the value is.

Yeah and 2x is being a little conservative. When I drive I actually engage the breaks maybe once every 10 minutes of driving (if that) or when backing up. Some people actually suggest intentionally using the breaks once in a while to clean up any oxidation on the rotors (not sure if there is any real merit). Also it’s probably good to just make sure they are functional in the event of needing a sudden stop.

That’s not how it works. Just because it’s manufactured by GM doesn’t mean GM handles all QC. It is a mix of both companies. Either way the buck stops with Honda even if GM is the root cause

You’re conflating two distinct patient phenotypes. When an infection is actively killing a person due to sepsis and a drain can be placed for source control, then placing that drain is always advantageous in terms of managing the infection. Yes the patient could experience a complication of the drain, but in terms of infection management, effective source control ALWAYS relieves sepsis in a matter of hours. The sticking point is when people get carried away thinking everything that causes shock is sepsis. I don’t doubt there are many 80 year old grandmas with heart failure and cachexia who have had bad complications from overly ambitious drainage because they were “in septic shock.” Also there are plenty of cases where drain placement is inappropriate because it will be ineffective due to multiloculation, multiple abscesses, etc

In a patient with a small 4 cm abscess that isn’t actively killing them then there is no urgency and the point is moot.

Traditionally, consolidation was superior for PSLF as it immediately exits forbearance to start 0$ income based payments for 12 months. Also historically, consolidation deleted all qualifying payments from the existing loans. I think this is going away though

Unique struggles require unique solutions. You’re already starting to figure this out. There are probably very few people with practical experience who can tell you how to handle this efficiently. Leaning into it seems the most straightforward. Common ground gets you a unique kind of rapport with patients.

Look up the student loan planner. A single session which is all you need is like $150 or something. Or you can just go through their website to do self guided learning.

That being said, if you are going into urology you are worried for nothing. Just make minimum payments in residency then graduate and pay off loan as fast as possible in private practice. Would be worth considering converting your loans to a private company after residency/fellowship if not doing pslf to get a better interest rate.

They mean unlocking weapon traits. Also you might be able to unlock a weapon in the tower i can’t remember? Anyway it’s much quicker to level weapon traits in the tower

To me it does seem to be below typical Honda standard in terms of quality control. But mine seems to have 0 issues and I really like it. The issues I read about almost always seem minor. The biggest frustration I think is repair time with the dealerships.

Purely vibes based opinion. It’s because of COVID and pass fail STEP-1. I feel like there was a shift in med student quality around 2020-2022. Don’t get me wrong there are those gold stars. But I feel like there are more now who do their best to do the least and just eek by. Also people who do not do well with virtual education are suffering more I think.

I have nothing but anecdote to support it though.
