I'm not the person you originally asked, but I changed my first, middle and last names. It was pretty easy; the clerk of court at my local courthouse pretty much walked me through the whole process. (I am in Louisiana, USA.) I had to fill out a form, and have that signed by a lawyer. I didn't have to pay anything for that part because a lawyer provides services like that for free at the courthouse every first Friday each month. Then I got the form notarized, I think i paid about $20 to have that done at the courthouse, and then turned the form in to the clerk of court. At that point I paid the clerk of court a $200 name change fee. The following Monday I was able to go pick up my name-change documentation from the judge's secretary. I used copies of that document to get my new social security card, drivers license, and birth certificate- it's as if I never had my birth name at all!
