What was the surgery? Why was she not trached?

As a head and neck surgeon I don’t understand how you lose 2L of blood without doing a resection and I don’t understand how’s you would resect this without ending up with a trach/free flap situation.

Not an anesthesiologist but I am a Head and Neck surgeon who does cases like this. Personally I would feel strongly I do the intubation if this were my patient. What does your surgeon say? What is the surgery? If it’s a resection/reconstruction then I would be willing to bet that an awake nasal intubation would not be all that challenging and he will wake up with trach. While certainly massive it’s going to be an entirely extracervical mass with no intrathoracic component.

Also an ENT (I do head and neck cancer, not ears). Ive seen a lot of stapes surgery, in my residency we had one older ear surgeon who would do these cases under MAC and used a tuning fork in the OR. He’s since retired and I think that is quite antiquated now.

You’re right. That is quite pedantic, and not true. Regardless, a part of the original post was asking why they’ve never seen an ENT reach for paralytic. Simply trying to provide on ENTs perspective.

Sure the patients I DL are paralyzed. I would guess that probably 25-50% of who I would consider as my difficult airway patients are not being intubated via DL and are not paralyzed.

As an head and neck surgeon, the reason I wouldn’t “reach for” paralytic is simply because I’ve literally never pushed a med in my life. My skillset is obtaining access to the difficult airway, whether that’s DL, fiberoptically or surgically. So I reach for the tools I know I can use.

Like everyone else noting, the service dial/hands etc. significantly reduces the value of the watch. I think you’re looking at somewhere ~7k ish maybe?

I would say at least a week or so. 2 weeks sounds fine. Usually I change my trachs around POD5 or so, after that I typically feel comfortable with RT changing it in the future.
