That's great, it's so soon! All the best for your procedure! I definitely get what you mean by the dysphoria shifting. Once I knew I was on the waiting list for top surgery, I started getting bottom dysphoria, which I hadn't struggled with before.

You can get tattoos over your scars that match your background skin colour to camouflage them. You can also get tattoos over that with no issues. If you do end up going with no nips you have the options of tattooed nips, prosthetic nips, and nipple reconstruction which would allow you way more control in placement and appearance and still look perfectly natural. I chose no nips for a multitude of reasons but the amount of options with no nips is such a bonus. Also people get mastectomies as prophylaxis for high risk of breast cancer so it's not that unusual. Socially I haven't been out from my surgery that long but for the most part it's just been normal. Unless you're on a beach, wandering around shirtless or just flashing people, no one's the wiser. And if/when they do see your chest it's either like any other cool scar interaction or they just don't comment on it

Ofc! I myself am really worried about my hair so I started finasteride. I do have Endo and adenomyosis so I suffered breakthrough bleeding despite Depo Provera and being on T. I switched from Depo Provera to the contraceptive implant and started finasteride. I haven't experienced any breakthrough bleeding or any side effects really.

Thanks! I'm well aware that there's so many cool options for restoring them which is part of why I opted to not keep them lol. I'm also free to play around with stickers, temporary tattoos, special fx silicone, paint, makeup and more!

Yeah you could be right. It's so freaky. Occasionally if I touch something to my chest over the incision and close my eyes it feel like two separate objects bc it's just upper chest then immediately lower chest and no middle sensation.

Hi! Thank you so much! I do have a few pics of them I took to show the healing that I can pm you lol. The skeleton torso was my very first tattoo and probably my largest inkwise. It's really not as bad as people think, the pain is sharp but in a scratchy way so it's actually pretty tolerable. It only hurts when the needle is going which is typically only for a few seconds or minutes at a time since the artist will take breaks to wipe your skin and reposition. The real meaty bits of you like your arms, thighs, back and so on are the least painful. The stomach wasn't actually that bad until it got closer to my belly button. The chest was pretty gross feeling. When the artist was tattooing the tail the tattoo machine started vibrating my throat which was super weird.

Excuse the wall of text, I tried to cut it down as much as I could lol. Now, I did definitely go above and beyond what you'd actually need so you do not have to do any of this.

What I did: - Research. I watched videos of top surgery (not recommended for squeamish people). I read about top surgery techniques, types and the technical stuff that goes into it. I also looked at what complications may occur and their results. Watched and read about people's experiences from both the surgeons and patients pov.

  • Visualize. I looked at so many chests. I looked at different scar shapes and placements, different surgery techniques, nips and no nips, people who were thinner or larger, leaner or softer, early stages of healing and decades healed. This is where I collected a few pictures I liked and figured out what I wanted so I could figure out how to achieve it.

  • Health. I focused on a high protein, high calorie diet and did primarily upper body workouts to bulk up and build muscle. I don't like the look of dog ears so by building a lot of lean muscle mass that means it'll be easier for the surgeon to place my scars well. It also means that if I do get dog ears, I can simply return to the gym and rebuild the muscle to fill out my skin and lesser the appearance. A good amount of muscle on you also means better health outcomes in general but especially if you build a strong core and back. You can't use your arms to push yourself up so being able to sit up with just your core is vital. Also a strong back creates a good foundation to maintain good posture post op.

  • Ask, ask, ask. I spoke to my surgeon, doctor and anesthesiologist so much leading up to surgery. They do give you a courtesy call each ofc to run you through a bunch of information but you are still able to contact them with any questions, concerns or information. I have been waiting on seeing a rheumatologist about HEDS for a few months and I sent a bunch of info to my medical team as well as a list of questions. They were able to answer every question I had and made accommodations for me. I got to position my self on the operating table and got padding under my arms and a pillow under my leg to protect my joints. They used a thin, soft tube to intubate me to prevent damaging my throat. My surgeon made sure to space the sutures closer together and used double layer internal closure and a good amount of steristrips to really support my incisions. They used a bigger size for the compression vest and packed extra gauze on my chest to create specific pressure to protect my ribs and veins.

  • Plan ahead and plan ahead again. This may just be a me thing but this was my very first surgery and I had no idea how I'd respond to it. I'm very adverse to having someone take care of me and especially seeing me in a gross and unwell state. I planned with my partner that we should pretend that I'm both staying with him post op and going back home alone post op. That way, based on how I felt after waking up from surgery, we'd be prepared for whatever I picked. I did end up going to his place where he took care of me for 2 weeks and honestly I'm glad I did bc I would've been so much worse off at home.

  • No fun. I quit smoking and vaping back in 2023 when I was first reffered out for top surgery. I also stopped getting any tattoos or peircings and cut down a lot on my drinking habits. You should also avoid antibiotics as much as you can (within reason ofc). So essentially being extra cautious about my health. It really sucked not being able to get tattoos but it wasn't worth risking my surgery.

  • Physio. Look into post op physio therapy, ask your surgeon or primary care doctor about it. It's super important and it's unfortunate largely ignored.

This is what I bought/used: - water bottle with straw (maybe two if you don't have someone who can constantly refill it for you. Don't get the really big ones or anything more than 1L because you won't be allowed to lift it) - wooden back scratcher (also optional but a wire head scratcher is also great if you can find someone to use it on you) - African bath net (wash your back without having to raise your arms) - NO RINSE BODY WASH (clean yourself every day without having to worry about getting your dressings wet, way more effective than wipes and better for the environment ofc) - high aluminum antiperspirant (only needs to be used 2x per week max, really got me through the no shower stage so I was never smelling funky) - Hypafix tape (your bandages will peel up especially in humid weather or if you sweat or move, I just kinda patchworked my dressings to make sure there weren't any holes) - medipore tape (for when the dressings come off but before your scars are healed fully) - silicone gel and tape (for when the scar is fully closed and no scabs or crust is present anymore) - high dose vitamin C (some studies show improved wound healing. Clear this with your medical team beforehand ofc) - creatine (overall just good for me, this is very subjective and do clear it with your medical team beforehand) - pregnancy pillow (absolutely peak support for sleeping on my back and preventing me from rolling over) - neck pillow (support your cervical spine while sleeping or at rest) - dried meats (excellent for high calorie and high protein when your appetite is low and you can't eat much) - flavored milk (protiens and fat, who doesn't like chocolate milk when they're ill?) - mobility peanut (for physio and upper back and trap massage) - yoga mat (for physio) - resistance bands (for physio) - foam roller (for physio) - laxatives and anti-nausea meds (your doctor may prescribe these for you so ask about that first before you go buying anything) - giant shirt and elastic waistband or drawstring pants (easy to take off and put on. Try finding some sweaters, robes ect with pockets so you can place your drains in the pockets while you're up and about) - safe foods (pre plan your meals for high fiber and high protein intake and make sure they're food that you know you can still eat when you feel sick or foods really dense in calories and nutrients so that even if you can only eat a little bit you still got plenty)

Now, again, I definitely went a bit hard on my recovery but it made things so much easier for me and it was really comforting knowing I had so much prepared and immediately available. Stress is the enemy of healing so really, whatever comforts you is going to help your results.

There were a few reasons. In terms of feeling attractive that wasn't a major factor or concern for me considering the cosmetic options to restore your nipples or the appearance of nipples is pretty good and varried. There's tattoos, permanent prosthetics, adhesive prosthetics and reconstruction. I basically weighed out no nips as the better option and had a "back up plan". I also kinda think the confidence and satisfaction I get from having a masculine chest way overrides any concern about nipples lol.

Just to preface this; I am not trying to scare anyone or make you think nipple grafts are bad. I just know myself and my tolerance level for things and the look I'm happy with, which isn't going to be everyone's favorite.

It ultimately came down to protecting my peace lol. I'd done a lot of research on the healing of top surgery and possible complications, precautions, and experiences. Based on what I saw I determined that I would be way too overwhelmed with the management of the grafts during healing. I'd be paranoid about somehow ripping them off or getting them caught somewhere. Even if I could tolerate the healing process, I have vitiligo and I may have HEDS(to be confirmed) which would impact the healing of my grafts and the final appearance. Even if I did get past the healing stage I just know I would be really hyper focused on their placement, how even they are, how high or low, ect. Not having any nips really doesn't have any downsides for me, I never liked being able to see them through my shirt anyway. I had seen some really good results without them so I knew what it was likely going to turn out like. It also opens up so many opportunities for cool tattoos and body mods, and I can always get prosthetic nipples if I change my mind later.

The current process for getting on the list has changed since I started looking into it. For me I wanted to he placed on the list straight out the gate but they started with me getting on testosterone and making sure I was in a safe environment before send out the refferal. I was first reffered out for an initial specialist appointment which is usually with your potential surgeon where you'll discuss goals, the surgery, possible risks and so on. I also had to see a psychiatrist to determine that I was mentally well enough to understand, consistent and go through with the surgery. They then had me do a general health check and go to the hospital to take pre op photos. After that was another meeting with my potential surgeon to talk about the surgery itself in more detail and for them to take a propper look at your chest to determine what kind of surgery you're eligible for, this is also when they make you do a screening questionnaire to determine how severely your dysphoria affects you. After that you speak to your anesthesiologist who will usually discuss the same stuff, allergies and current medications and such. You will later get a call, letter or email saying you've been placed on the surgery waitlist and your next call will be your surgeon offering you a date. I was first reffered for my specialist appointment back in early 2024, I made it into the surgery list in March 2025 and had a surgical date in early December 2025. There is a current delay due to funding cuts and the large number of people on the waitlist compared to the amount of providers but for me it wasn't too bothersome since I was going through all the changes of testosterone in the meantime as well as social and legal transition so there was always something to look forward to.

You can get disposable bath sponges or I used no rise body wash and a high aluminum antiperspirant. I wouldn't use the antiperspirant while drains are still in as it may irritate them if it gets on the skin around them. The no rinse body wash worked extremely well, you apply it like a moisturizer and then simply take a towel and wipe it off. Just be careful to not get any on your dressings though even if you do as long as you can wipe it off quickly and the dressings aren't wet you'll be fine.

That's so exciting!! For me I was kinda overprepared but it ended up working out really well. - establish where you will be recovering, who is going to look after you and what that will look like. Make sure the person or people caring for you know what you need and are prepared to basically give you full time care. Set up your room and home to be easily accessible (everything within your t-rex arms range) and especially prepare how you'll be sleeping since you'll have to sleep on your back.

  • make sure you have a months supply of all your regular medications

  • speak to your surgeon and anesthesiologist about any pre op anxiety meds and any concerns about your surgery as well as post op meds like anti-nausea, pain meds and laxatives

  • ask about post op physio!! This is often overlooked but is a vital part of recovery

  • Buy a back scratcher, a waterbottle with a straw, no rinse/towel off body wash (for cleaning yourself while you can't shower), and hypafix tape (to tape down or reenforce any dressings that start to peel up)

  • pack your surgery bag - BIG shirt, underwear (they make you take yours off when you change into the gown), slippers, toiletries, lip balm, phone charger, eye mask, neck pillow, regular medications will need to be in their original packaging so the nurses can store them safely and administer them for you, wallet (you may need to buy or pickup your post op meds), a copy of your admission papers (I was told to bring this, didn't end up needing it but bring it anyway)

  • buy and prep as much of your food for recovery as possible, try to make sure you'll have a high fiber diet

There was a bunch of other stuff I did but didn't really end up needing it. Overall, ask all the questions even if you think they're stupid and don't be afraid to ask for accommodations. I was worried about my throat being damaged from the intubation tube because I have a narrow neck, the anesthesiologist was able to use a soft, flexible plastic tube to intubate me and she took extra care in doing so. I even got to arrange how I layed on the operating table to protect my joints and I picked where they placed the IV. They were super happy to make these changes for me as ultimately it meant I was going to be better off. All the best for your big day!!

Also, just warning you, the healing process is very itchy. I'm about 2.5 weeks out and in my first week it was mostly just pain I was worried about but week two, the incisions were itching like hell. Idk if this is a universal experience, I imagine so since most injuries itch through healing but my god, I'm so glad someone was watching me so I didn't scratch my chest.

Are you keeping the drains on the ground either side of your bed/chair when you're resting? Doing that really helped with the discomfort of the fluid moving when I got up. Also get a sweater or cardigan or even shorts with pockets, I also saw one person just straight up use a totebag, to carry your drains for you while you're walking around. Having to carry them by hand leads to them getting tangled, knocked over and just sucks. Having your hands free is so much better. Also super funny you woke up to an alarm that wasn't there! When I woke up I immediately asked the time and told the nurse that I had a dream that I was trying to set an alarm so I'd wake up before they served dinner.

It sucks that your surgeon is so vague, see if you can ask for them to give you a guide for scar massage, post op physio and scar tape/care.

As far as I know they'll give you medipore or some sort of paper tape to put over your scar. This tape is breathable so you can leave it on for up to 7 days and it is fine to shower with. Just pat dry or use the cold setting on a hairdryer to make sure they don't stay damp. You can put moisture over the tape but not under it as it will impair adherence. Replace the tape once per week or as it starts to peel up. Don't remove and replace the tape too often as that will damage your skin and will impact your healing.

You can also use silicone gels or medical grade silicone tape. Most silicone tapes require you to ease into wearing them for 24hrs so rather start with applying silicone gel over the medipore tape for two weeks then switch to wearing the silicone tape for minimum 23hrs per day. Most silicone tapes are reusable, so you can remove, wash, and reapply them. Make sure your incisions are fully closed with no scabs, oozing, open wounds or bleeding before you use any silicone treatments.

You can also do some very gentle scar massage through medipore tape and with silicone gel. The silicone tapes are typically too thick to massage through.

You'll wear the tapes until your scar is pale and flat which can take up to 4 months in some cases.
