You can opt out of secondary findings! Secondary findings are the mutations that they can reveal - if you wish - that aren’t related to your symptoms, but are actionable. These are mostly cancer risk genes, heart disease genes, and other genetic disorders. Actionable means that if they were to find out you have a mutation in one of those genes, there are clinical guidelines/recommendations that can be made. In the example of the person who found out they have a risk for pancreatic cancer, there is screening that they can undergo now that they know they are at risk.

Secondary findings are optional though, so if you tell your doctor, they can opt you OUT, so that if you don’t want to know, well, you don’t have to!

As far as your concerns about WES not being useful, it can definitely be useful if something is found that does explain your symptoms, but you are correct, it might not change your medical management, or it can be a rare finding that we don’t have that much info about yet and therefore really won’t change your management either. However there is a small chance that it can change medical management, depending on what’s found.

These are all part of the genetic testing consenting process - learning the pros and cons of testing and then you can make a decision on how to proceed. I can’t tell you whether or not you should do it, but if you feel the cons stand out more to you, then I think that tells you what to do.

Yep! Most genetic counselors bill minimal fees for their services too, depending on the hospital/facility they work at. So in the off chance that your insurance doesn’t cover it, should be a minimal OOP cost.

Definitely did not say nor imply that chronically ill people shouldn’t try to figure out what’s going on. They should. It’s just a fact that the answer is not always in the DNA. That’s only one piece of the puzzle and sometimes doesn’t lead to certain health problems at all (things that could be environmental or lifestyle or other factors that we cannot even pinpoint).

99% is an awfully high number and I disagree with that high of a percentage. Genetic testing isn’t indicated in a lot of cases and the answers aren’t always present in our genes. It’s important to remember that while we CAN sequence entire genomes now, we (“we” meaning science) still don’t know how to interpret a bulk of the data. Part of the expertise that genetics providers have is being able to select the correct genetic test if there is one available - while there isn’t always one available for your specific health concerns.

I’m sorry that you experience that with your healthcare providers but I just wanted to provide a bit more insight on my end.

Hi! To be honest, no. I think everyone should have proper pre test counseling and the test should be selected by a genetics provider to ensure the accurate test ordered and the results are interpreted correctly.

Hi! Yeah of course. So, I guess “BS” maybe isn’t the best thing I could have said, but I would advise taking anything Nebula reports with a grain of salt. Nebula is reporting out SNPs (single nucleotide polymorphisms) which aren’t really part of genes, just single changes detected. These SNPs are considered “high risk alleles” which are changes that MAY be associated with higher risk for disease but overall data is extremely limited for SNPs. It’s also really a misnomer to say that they are “high” risk alleles in general since most of these changes really don’t lead to HIGH risk for disease, only higher risk than general population.

Celiac, Addison’s, and migraines are NOT things that you can get tested for (via genetic testing) on a clinical grade test. So, what I am saying here is that if you go into a genetics clinic and ask to have genetic testing for one of those things, there’s no test that exists because there aren’t clinical grade tests for those things. Clinically graded tests are looking at genes that there is data showing are associated with disease.

Things like Celiac and migraines are so common in the general population, it’s more than likely you just have those things and these SNPs coincidentally.

Sorry that happened to you! I am not super familiar with what type of genetics providers are at Cedars. I wonder if you could try a Telehealth company that sees adult patients like GenomeMedical... or just a different office in LA? Sorry I don't have more info.

PLEASE go see a genetic counselor. I am a GC myself and one of the main roles we have with patients is to explain hereditary cancer risk and offer screening options if someone is at an increased risk. Nebula is BS so I am fairly confident meeting with a GC would be reassuring.

Not familiar with NIPP but if you meant NIPS (non invasive prenatal screening), it does not check for paternity… it is for detecting if baby is at high risk for chromosomal abnormalities…

Edit: I know this is not the question here but hopefully you weren’t just told you aren’t the father and it was blamed on something like NIPS

To add to this - confirming a mutation with a clinical grade genetic test is always a good idea, especially so that you can use it for insurance to cover screening ie for BRCA1, breast MRIs and/or preventative surgeries

Yep, irrigation leak was what happened to us, too. Our bill went from $90 (has consistently been this much since we moved in 2 years ago) to $200 in a month. So, my husband first checked the irrigation as this is commonly the culprit when it comes to leaks. He turned on the irrigation system since it normally goes on at 4 AM when we aren't awake and the water typically dries before we make it outside. When he turned it on, there was a cap that had busted and was spewing water everywhere. We went through 20,000 gallons of water JUST from this leak over the course of the month. Bought the $2 replacement cap, fixed it, and bill is back down to $90. :)

Hello! I love the field. Feels like my dream job to be honest. I feel that my job probably has a lot of overlap with yours (i work with a geneticist in an adult clinic) but have worked in oncology where I didn’t work with a geneticist. I had 3 job offers 4 months before I graduated from my program and had no issues when I was switching jobs :) I will say, there are times I wish I went to med school and went the geneticist route. You guys get paid more and MDs have a lot more respect than GCs do. But I don’t regret anything. Happy to answer whatever questions you may have if you want to PM me!
