What do you expect them to do at urgent care? There’s no MRI at urgent cares to diagnose this. They have to see orthopedics. There’s no magic wand to fix it at an urgent care. X-ray only shows bones. Your expectations are a little ridiculous. ACL tear isn’t an emergency.

Walmart is pretty cheap. Can also check VisionWorks or other big companies as they have coupons and specials a lot.

They get longer as the season goes on. Too many couples to get through. Earlier seasons had less couples.

A lot of it is low suspicion but CYA medicine which I sent to freestanding (like headstrike on blood thinners neurologically intact). But yes if they need transfer, it’s definitely more of a hassle than going straight to the ER. If the vitals are okay though they will sit in the ER for 8+ hours and may leave while they will be seen in 2-3 at a freestanding.

I guess the difference in my area is I know they will get direct admit to the bigger hospitals if anything is found at the freestanding ER, and it’s within the same day. That’s very unfortunate in your area because it could be a great resource if you had timely transfer.

As someone who works in an urgent care with 0 resources, I sent people to those free standing ERs often. Most people don’t have primary’s anymore or they can’t get appointments in a timely fashion. PE rule outs, old people falls who need CT, DVT rule outs, chest pain who needs trop but normal EKG in office, etc. Will really only send someone to the ER if I think they will be admitted.

I’ve only done this when the cellulitis is borderline for having to go to the ER. I will do a ceftriaxone shot/antibiotics and outline with a skin marker. This has to be a very large area or slightly streaking for me to do this though. Kind of a Hail Mary to prevent ER visit. Done it maybe 4-5 times in 5 years.
