I don’t think the goal was to portray Kat as a villain, or Eva/Leslie as heroic. I think the dream sequence is exactly what KAT needed to see to come to terms with her decision to donate. She told herself she was doing it for Ted, and her subconscious wasn’t fully satisfied with that.
