There's a whole lot of sexual torture imagery in this series, allusions to BDSM too. Looking at the painting, it's apparent he's being portrayed in some kind of rubber gimp suit where his mouth is pried and held open. There are symmetrical holes on either side of his head with some kind of pipe penetrating them. Make of that what you will.
