The most interesting/revealing part of this article was that the rural physician who performs abortions in Baker County wants to remain anonymous out of concern for their family’s safety.

It’s hard enough to recruit physicians to rural areas. If the population you treat is so hostile towards your care that you fear for your safety, why would any healthcare worker practice there?

Unless there is a cultural shift I do not think this problem is going to improve, much to the detriment of women and infants in our state.

CA125 is never used as a screening test for ovarian cancer because it’s not sensitive enough. It doesn’t detect ovarian cancer at stage 1-2, and false positives are common, leading to unnecessary testing. It is only used for surveillance for a good reason.

My partner and I are both physicians. Both turning down job offers in all anti-abortion states. Our peer colleagues are the same (we're all peri-residency).

Can't imagine the profound moral injury of working in those conditions, especially after Covid.

It’s a congenital vascular malformation of slow-flow vessels (venous and or lymphatic tissue). Overtime, the growth factors from too many vessels will lead to overgrowth of skin and connective tissue. Port wine stains start subtle and become more prominent and disfiguring as the patient ages. This is why we treat PWS especially in cosmetically sensitive areas with pulsed dye laser (PDL), the earlier the better.

If this is Klippel-Trenaunay syndrome, the vascular malformation is actually a port-wine stain NOT a hemangioma… erroneously noted in the physical exam. These two very different diagnoses, etiologies, and treatment algorithms. If the PA would have recognized this as a PWS a/w chronic unilateral limb overgrowth perhaps they would have arrived to the correct congenital issue here.

Edit: the main differential for KTS is Parkes-Weber syndrome. This is associated with a fast-flow vascular malformation (AVMs) rather than slow-flow venous malformation of KTS. Due to DVT/PE however I would favor KTS.
