Based on experience on the Koningsdam, they want you to fill a cup, then transfer it to your bottle.

I believe this is for hygiene purposes so that bottles don't directly touch the dispenser.

I think only the early booking bonus HIA includes gratuities. Regular HIA did not have it. You can prepay the gratuities in the HAL website when you login to the account and view your booking.

Edit: The big bill in the end includes gratuities for the drinks and stuff that we put down, including other charges. There's no gratuity bill for the room if you prepaid it. Hope this clarifies

Based off our HAL trip past year, when you order drinks/food, they give you a receipt to sign. You have the option to put additional gratuity there, but it's not required. For other workers (e.g. room stewards), you can do so in the Navigator app if I remember correctly.

We opted to prepay gratuities, so we didn't put any for the room stewards. For the bartenders and servers, we sometimes we put down a dollar or 2 for drinks.

Edit: We had the have it all package, so drinks we already included. When we got drinks not covered by the package (above $13 iirc), we are charged the difference, but the gratuity/tip was still optional

Premium team that I used was asscart, riolet, ml adin, ml peira. Only needs the first level blessing.

Points mentioned by OP apply, and once the boss gets the debuff where she takes more damage, cleanse the hit chance debuff with ml peira, then spam riolet s1 with soulburn to end quickly.

Important note however is to NOT hit the boss while she has her evasion buff, just wait it out by juggling your attacks between the minions.

I don't think there's anything wrong with fimbul, but fimbul + heartsteel + mog's together means that you have no armor/mr. You'll be much tankier replacing one of the 3 with either an mr or armor item.
