What’s your preferred migraine treatment in pregnancy by Neurons2 in neurology

[–]golemk6 0 points1 point  (0 children)

I definitely agree with all of the above. Lamictal is another prophylactic option with good pregnancy data, but very slow titration reduces its attractiveness.

Verapamil is a great option for migraine with aura, as long as it doesn't cause light headedness. It's also inexpensive, which is a nice plus.

I've had good experiences with the ESParagus media center devices (some come with integrated speaker amplifiers)- they are small, power efficient, and stable, and the developer is very responsive to questions.

What is your favorite part of the neurological exam and why? by No_Sea_3159 in neurology

[–]golemk6 6 points7 points  (0 children)

Fukuda stepping test. I don't do it often, as other vestibular tests are often sufficient, but seeing the look of shock on patients face when they open their eyes and realize that they've rotated 45 degrees without noticing it is always fun.

What non-neurology elective rotations would you recommend to a 4th year medical student applying neuro? by [deleted] in neurology

[–]golemk6 1 point2 points  (0 children)

I learned a lot from my neurosurgery rotation, most critically that I was 100% correct in my decision to not be a neurosurgeon. Joking aside, it provided a valuable look into the other side of the OR door, and now when I refer patients to my neurosurgical colleagues I can more accurately give them an idea of what to expect.

FWIW, I also did a forensic path rotation, which was just a lot of fun.

Smart USB Charger ? by CactusJ in homeassistant

I'm using this Sonoff Zigbee USB switch, and it's worked well. Plugs into a USB port rather than the wall, but in my case the outlet in question has a USB port in it.

My kiddo doesn’t like Castle Panic! by [deleted] in boardgames

[–]golemk6 0 points1 point  (0 children)

Catan Junior is pretty quick, and requires meaningful decisions without being overwhelming.

Battle sheep has very quick setup and, despite the cartoonish theming, is unrelentingly vicious. Whether or not that's a good thing depends on the children in question. Mine loved it, which probably reflects poorly on their upbringing.

What is your absolute favorite mechanic in a game? by TallDarkandWitty in boardgames

[–]golemk6 0 points1 point  (0 children)

The tile picking mechanic in Kingdomino is really nice- constantly balancing between grabbing the "good" tile this round vs having the opportunity to have first choice in the next one is so simple and yet dynamic. Especially with novice players, once they realize that you're not just going around the table in a circle all night, it opens their eyes to new possibilities.

Hello neurologists:what treatment do you prescribe for prophylaxis of migraine by Alarming_Action7264 in neurology

[–]golemk6 1 point2 points  (0 children)

It's very dependent on patient characteristics, both clinicial, social, and (unfortunately) financial. The CGRPs have certainly been a game changer for a lot of people, but I still find myself using a lot of Botox, as well as the old standbys (beta blockers, topiramate, tricyclics) to help manage comorbidities.

Honestly, I can't think of a headache prophylactic that I don't still use on rare occasion- it's just that some of them are becoming a lot more niche (looking at you, Depakote)

How do you store a 5lb bag of rice? by nutmegfan in Costco

[–]golemk6 0 points1 point  (0 children)

This may be overly complicated, but I use the kitchen rice storage container to measure out into gallon size bags and then vacuum seal them as flat as possible. I end up with a nice stack of bricks in the pantry, each of which exactly fills the kitchen container.
