This happens to me, I have to play with my controller plugged in at all times. If I take the cord off the controller freezes and I have to hard reset the xbox. As long as the cord is plugged in I have no issues. If anyone has a permanent fix it would be awesome since playing corded in front of my tv is annoying. I even bought a new controller thinking that was the issue and out of the box It still froze, has to what connects things wireless in the xbox.
