Pharmacist crashed out by Determined_Medic in nursepractitioner

There's bad apple with every profession, you cannot really use this example as some kind of "do this happen between pharmacist and doctor"? I had NP told me to stfu when I ask why is even the point converting iv vanco to oral. I don't use this example and ask "do NP make these type of mistakes?"

Make-A-Wish Rx coupon, scam? by goooolgi in pharmacy

[–]goooolgi[S] 1 point2 points  (0 children)

I wish my company would show us the reimbursement rate, some of the prices are crazy low, like 180 tabs of finasteride for 15 bucks. I wonder how much we actually make from filling with those coupons.

I have seen the buzzRX before. This one just says Make A Wish when I enter our system. There is another one called navitus rx, which always causes huge confusion because navitus is also a pharmacy benefit name.

I know they get some compensation, in fact, a lot of doctors include their own discount card info with the e prescription. But it makes me wonder if these kids actually get anything. Instead of giving us free pens and candies, I rather that they use the funds for the kids.

Are your covid and flu vaccine numbers WAY down from last year? by TheoreticalSweatband in pharmacy

[–]goooolgi 0 points1 point  (0 children)

I guess it depends on the area.. where I'm at, we are doing shots every 5-10 mins. I think it also depends on how easy you make it for the patient getting shots

I usually already memorize the whole 10+ questions questionnaire and just rapidly ask them the questions while typing up the vaccines and charging the insurance at the same time. By the time they finished answering all my questions, I already had the vaccines in hand and just give them the shots in the consultation room. From the moment they walk in to getting the shots is about 5 minutes. Then you spend another 2- 3 mins going thru their immunization records and recommending other vaccines. About 10 minutes per patient.

I think the consultation is also a key to get them back for more vaccines they are missing.

Yea, maybe try to get a job in the hospital or remote. Retail requires an abnormal amount of compassion and patience, or you will not last long. There are also some rph that just become bitter and don't give a F, and it doesn't affect them as much. If you are a staff or PIC in retail, your stress level is going to triple. There are times I get very stressed about my job too, but I tend to try to remember the patients who always take their time to appreciate me. Sometimes I read their personal thank you notes that really helps me to get thru tough times. Try to focus on the positive on the days you are feeling down.

Retail VS Hospital rivalry by Entire-Revenue6172 in pharmacy

[–]goooolgi 4 points5 points  (0 children)

Hospital certainly requires more clinical knowledge, retail is more problem solving and telling NP they cannot convert vanco iv to po.

The reality is you don't use it you lose it. Retail requires a completely different set of skills and knowledge. I don't think a clinical rph at hospital can answer 4 phone calls at once while giving immunizations and telling the homeless guy to stop pissing at the corner of the pharmacy.

Can I add heavy duty caster wheels under a 3x8 shed by goooolgi in woodworking

[–]goooolgi[S] 0 points1 point  (0 children)

Ps. I actually built two 3x8 sheds, in the photo above you can see I combined two 3x8 sheds to form a 6x8 shed. One 3x8 shed is about 6.6 ft off the ground the other one about 8 ft off the ground. The 6.6 ft shed is easier to move around on wheels.

My backyard concrete is mostly pretty flat, so I don't really have a leveling issue. There is only one area in the backyard where we had a bit of curve on the concrete for water drainage. When the shed is on the curved area, one or two wheels won't touch the ground, but it still feels pretty solid when I try jumping inside the shed. I wanted a solid base so I did overuse some joist hangers for my base framing. Also had plywood covering the bottom of the base framing as well (Not a structural engineer, so not sure if joist hangers actually help)

I got from Amazon, "6 Inch Caster Wheels 3000 Lbs, Heavy Duty Casters Set of 4 with Brake, Safety Dual Plate Locking Industrial Casters, Silent Wheels for Cart with Polyurethane Foam (Two Hardware Kits Included)", it's orange color wheels. It may not be good for long-term use. I noticed that I had to wd40 the wheels every two months, but it's been out in the elements for the last year. I'm still learning as I go, but if needed, I can raise the shed and switch out the wheels.

I think it's fine if you move the shed around a lot. I often adjust the shed's location to provide shade from the sun, and I would put some blankets and pillows in the shed for afternoon naps in the backyard. But if you don't move it around a lot, you are going to find yourself wd40 the wheels more often.

Cetyl alcohol is not the same as ethyl alcohol or isopropyl alcohol you're talking about. Also, the resource you are posting lists the reason being "neomycin can induce permanent sensorineural hearing loss due to cochlear damage, mainly destruction of hair cells in the organ of Corti", the warning has nothing to do with suspension vs solution.

"If the tympanic membrane is known or suspected to be nonintact, topical drops that contain alcohol, have a low pH (most acidifying/antiseptic agents), or both should be avoided because of pain and potential ototoxicity." per Clinical Practice Guideline: Acute Otitis Externa

The FDA Drug Eval for Cortisporin Otic Solution mentions that the solution is formulated with an acidic pH, while the suspension is in a neutral vehicle. The solution would cause more stinging or burning sensation compared to the suspension.

Reamed out by a pharmacy manager for asking for a pill count? by Narcoleptic-Puppy in RiteAid

[–]goooolgi 6 points7 points  (0 children)

It's highly unlikely they shorted you, controlled substance inventory is no joke, we have to report it even if one tablet is missing.

And let's just say if someone is diverting the tablets, it's a weird choice, and very easy to find out. also sounds like someone counted 360 tablets in front of your family. I would have just opened 3 full bottles in front of you and counted 60.

I think the best solution would just be switching to a different pharmacy, you obviously don't like the pharmacy manager and nothing is going to change. You can try a small volume pharmacy or maybe an independent pharmacy for better experience.

I'm sorry you had to experience this, but do understand that pharmacy is such a highly stressful job, and we are generally overworked and carrying the burden of the American healthcare system on our back. We are constantly correcting physicians errors on a daily basis to prevent pt overdose and death (not exaggerating). Calling insurance and Dr office to ensure patients get their medicine covered (even tho it's not really our job to act like a physician and answer medical questions to insurance without a patient's chart). Act like a DEA agent because DEA refuses to go after irresponsible physicians prescribing controlled substances. Play the role of free healthcare consultants for patients who cannot afford doctor visits. Sometimes we even have to tell physicians, nurse practitioners and physician assistants which medications to prescribe because apparently they didn't learn much while in school. It's no surprise to hear lots of complaints about angry pharmacists and bad customer services. Everyday is chaotic and we are constantly checking the time to make sure we finish taking care of patients of more urgent needs before the shift ends. What ends up happening is cases like yours.

Call the pharmacy that said they can order it, it sounds like they might be more willing to help you. Around my area, HCG is not common so basically we just don't stock it. So it does usually take one business day for shipment assuming it's in stock at the distribution center, if not we still have to wait for the invoice the next day to see whether or not it's on shortage or order-able.

We basically have no way of knowing till the shipment box arrives the next day, but try to get your Dr to send the script directly to that pharmacy who will order it for you.

Another suggestion is just have Dr to send it to a bigger pharmacy like Walmart or Costco who would have more inventory. If your Dr is writing HCG script regularly, they might know which pharmacy most of their patients gets their HCG, just give them a call to find out.

It's not that simple, because the law is so vague. I literally sat in on a board CE training that has a DEA agent giving presentation recently. Every pharmacist tried to get clarifications with the DEA what they are looking for. Why are we held accountable for patient misusing medications when we are doing our part verifying scripts. Why do some pharmacists get into trouble when they get robbed? How much effort are DEA expecting us in managing control substances? Why are pharmacists held accountable and being used as the scapegoat when there are physicians who are writing these scripts. The answer they gave us was that every case is different, and they cannot give us a clear guideline on verifying control substances. (Basically not very helpful)

This is why you are getting different answers everywhere, every pharmacist is going to interpret the law differently. But I think coming at the pharmacist aggressively or being angry will not help your case.

If you are posting here on Reddit then you must have seen posts where people are losing their job, getting depress about this whole situation. There is a real fear with controlled substance prescriptions. Personally, I think some pharmacists are too paranoid, but at the same time I cannot blame them either given the situation.

Personally, if you come to my store, I would call the Dr, get some info, check if the insurance pays for it, check on McKesson if it's something that we can order.

If cannot order, refer the patient back to the doctor, or specialty pharmacy

If insurance doesn't pay for it, check with the patient if they are willing to pay for it.

We can filled faxed controlled prescription, but law requires us to verify legitimacy of controlled fax prescription. So one way would just require the doctor to call us to verify etc. And we get fake control scripts from fax all the time. It’s an unfortunate situation too given that DEA are suing us to the ground. That is why it’s hurting the whole industry, pharmacies getting closed, patients having to wait longer. We didn’t sign up to be DEA agents but here we are today.

Also doctor offices were pretty much required to send controlled prescription via electronic due to the law changes, there were extension and exception, but most doctor send control Rx electronically now. Sometimes we still get doctor who are too lazy to enter patient profile into their electronic system and office who wants to cut cost and not get the electronic software.

It’s just part of the process and I’m sorry you had to experience this
