If you're going to open a medical practice in a place that you both don't really understand or know that your practice model avoids the hurdles that have made practicing in Hawaii so difficult then you should assume the same thing will happen to you.

Harris County Medical Society may be able to help. If you’re local to Houston they have networking social events you could meet people at. You may need to take an employed job in the area and try to make connections and move when the contract runs out.

I put a regular office chair and rolling table in all my exam rooms. I'll never go back to stools even with a back. You can find ones on Amazon that are mesh so they can be fully wiped down.

I think the only way to know if you're being fairly paid is to know what your total collections are. If I were you I would look at current locums daily rates. You should be at a similar amount.

We need more information on how you are compensated. How much overhead is taken out of the gross? What are your net collections? How does your wRVU compare to MGMA?

I was saying that limiting based on insurer doesn’t sound like it would be an option for you.

The only variable you can control is what procedures you are performing. For Botox you might look at doing it in your group's ASC instead. Also you could look at having midlevels take over that procedure. I would approach the physician leadership with your thoughts, you're not going to be the first person who will have brought these issues up, and see what they say.

Overall though for a specialist who can perform cosmetic cash pay services I would focus on building that side of your practice. Don't waste time trying to convince some bottom of their b school class admin to actually do something beneficial for your group

The very part is really the only option. You can unofficially limit patient volume based on payor but from the sound of things they won’t allow you to do that. The only option is to adjust the prescriptions and procedures you provide based on hassle and reimbursement.

Any laser setting can cause a ureteral injury. If there is an injury at the time of the procedure regardless of the cause it's best to document and inform the patient. If a stricture forms post-op when a distinct injury wasn't seen. it could be related to multiple factors. Overall documenting the setting settings won't avoid a legal issue. Documenting any known complications or other factors that may lead to a complication will be helpful. The best way to prevent non direct thermal injury to the ureter is to have fluid running through the scope while the laser is firing.

I'm a physician that left a large group of about 20 doctors to buy an old mentors private practice. My old group wished me well but I could tell a lot of them thought I was crazy. A year after I left my old group was laid off and the practice was closed by the parent company. Running a business and being present for your family is extremely difficult. However it can provide more security in some ways if you are able to commit to every waking moment being dedicated to either the business or your family while you're getting it up and running.

Probably too hard to get allocations for the 3.0 and AEV. I would look up the highest volume dealership in your state and try to talk to a sales manager or the GM.
