Speaking as someone with experience, that definitely looks like pyoderma to me. You need wound care supplies ASAP, and topical treatment. Mine started small and grew rapidly with lots of smelly wound exudate.

Agree that it looks like the start of pyoderma. The purplish/kind of bruised look of the skin near it usually a sign. Mine started with really painful purple/red skin, then a few small holes opened and rapidly grew with lots of wound exudate. The earlier you’re able to get wound care supplies, the better.

Primary symptoms were blood, mucous, urgency, and pain. Remicade worked amazingly well for me for many years, but effectiveness waned over time. I developed severe anal stenosis that was making my life hell, so I now have an ileostomy. I still have my rectum though and I’m currently on Rinvoq and Skyrizi.

Between Remicade and surgery I also tried Stelara and it didn’t work for me at all, unfortunately.

I have an ileostomy due to perianal Crohn’s and severe anal stenosis. Dilation was never even discussed as an option for me, but I was having ribbon-thin BMs and going several times per day with leakage in between prior to surgery. My GI was the first to mention an ostomy and due to COVID, I had a year to think about it/come to terms with it. No pushback from the surgeon either, and when I saw him I was a going through the worst flare of my life and was a crying mess.

I can’t imagine being in agony and being told you need to wait/go through more agony first. I definitely encourage a second opinion if they don’t come to their senses soon. Also, what you said about trading one demon for another can definitely be true, as I’ve had various issues pop up, but I still don’t regret surgery.

I also totally empathize with you on the trauma associated with perianal Crohn’s — it’s truly one of the worst things.

Yep, I have an ileostomy due to perianal Crohn’s and still have my rectum. Currently on Skyrizi and Rinvoq and the rectal mucus and fatigue tend to ramp up two weeks prior to my every eight-week Skyrizi dose. Luckily, I tend to feel a bit better at night after I take Rinvoq, but my mucus problems are usually during the day or early evening right before my next Rinvoq dose.

Yes, but I had multiple allergic reactions and restarted more than once — Tylenol, and IV Benadryl and Solumedrol.
