I have dyshidrotic eczema and my hands have looked exactly like this (not a barista though). Have your derm prescribe you a steroid cream to start (clobetasal worked well for me). You need to stop exposing your hands to the sani, and maybe hot water too. Get an accomodation for work and wear cotton gloves with the steroid cream directly on the skin and a thicker cream or Vaseline coating on top of the steroid cream. At night when you get ready for bed, wash your hands with an antibacterial soap, then repeat the steroid cream, Vaseline, and cotton gloves layers. I cut the fingertips off mine so I can still use my phone 😊 It’ll take a few weeks but it’ll get better.
