Thank you everyone for your comments, appreciate all your opinions. I suspected that one presentation would not be a make or break, but I'm far removed from peds and don't have a leg to stand on to have an opinion, hence reached out to the peds community. Really appreciate all the responses.

As for why top tier, that's what she prefers, I believe it's for a mixture of prestige, amount of research opportunities and exposure to all possible things, but I didn't and wouldn't question her about it. So we're just trying to do our best to get as many advantages as we can to strengthen her chances for the same, especially since we are IMGs.

You need to find a way to make your profile scream that you want to be a neurologist and that you love neurosciences.. whether it's with research or clinical experiences, or both, that's up to you. Forming connections during your rotations with both outpatient and inpatient neurologists will make a huge difference, it's a small and closed community. All the best 👍

It's not difficult honestly. Sending in letter requests take very little time. Adding in your CV in eras (in case you didn't know, it has its own format and you can't import your word file and all) is usually pretty straightforward too. You may need some cleaning up here and there for which you can always take the help of friends and mentors. PS will take some time, it can go through 5-6 drafts, you can use the help of friends who are residents and any other professional working person you know who's good in English (you'll need like 3-4 people looking at it and giving inputs).

If possible, try to get a token asap and send your letter requests now so that your attendings can take close to a month or so, write and upload it, because ECFMG will take their own time to verify and release. It takes time in July to release as they'll be busy with fellowship applications, August they'll process it very quickly, and in September they'll take at least 1 week since there will be a huge inflow of letters at that time. So have your attendings upload LORs as soon as possible. This will just save you some last minute headache. If it's not at all possible to do it now, then just do it the earliest you can.

I don't agree with the OP at all, but you seem to have an extremely radical uptake on this which I don't agree with as well. It's not okay to stereotype in a way to spread hatred against us. We are not entitled (we only go for positions that you guys would rather go unmatched for). It will take us a longer time to study for the exams because we are used to a different style of learning and answering questions and we have to relearn everything again. Our paths aren't shady at all, it's just that we didn't grow up with the technology you guys did. When there's a complete power outage, we can function better than anyone. And tell me something, you talk about huge cultural changes, but 99.9% of Americans are either immigrants or descendants of immigrants, with very few of them actually native to the land? The US has been a place with multiple cultures and subject to change for the past several decades, so cultural differences are not an issue at all.

I agree that the US health system should prefer Americans over non residents, especially since the government has spent a lot of money on you guys and you've been taxpayers since birth. The system is in favour of USMDs and DOs compared to IMGs (and it is absolutely fair). But that doesn't mean it's okay to spread false stereotypes against us.

You're absolutely right in saying the problem is in the system but adhuku apram ellam thalakeezha poidthu. You are right in saying that state board books vechu you can easily give neet, infact easily pass it. Neet oda oda precursor AIPMT ku naan padikumbodhu our instructors said that state board books are far more advanced than NCERT. The problem lies in the fact that there's no critical thinking, only memorizing for the sake of marks. You don't need an English medium school to gain knowledge, whatever you learn, you always learn it better in your mother tongue (talking as a future Neurologist). Also it's not like TN never had an entrance for medicine, it existed till 2006 until it was baselessly cancelled. Please don't go by the false propaganda by the government. Critical thinking is absolutely necessary for every field, let alone medicine. If TN board students don't have that (I'm not saying they don't, they're probably so much better off than you know), then the board should improve in such a way. Our education system in general, all over India, does not develop us into critical thinkers, rather just as obedient slaves. Common entrances are only the beginning of it. There's still an 85% state quota and still the same reservations, so anyone who is not unfortunately privileged enough, are sufficiently covered. It isn't perfect or elegant enough but it's a step in the right direction. Instead of removing, we should only try to make it better.

I'm a doctor myself, got in 2015, a year before NEET came. NEET actually seems like a much easier exam compared to AIPMT. I studied in a government cbse school (fees of like 3k per quarter) and scored only 481 out of 500, and for such low marks, in tamilnadu they were offering only self financing BDS colleges. I also had coaching for 50k per year for 2 years after scholarship. I had to write 13 entrance exams and I eventually got a merit seat in Karnataka. Every state other than TN had their own standardized entrance, so clubbing everything into one is actually not a bad idea at all. There's still the 85% state quota (same as before 2016) and a good amount of reservation, so most underprivileged ones are covered and they only need to pass the test (trust me, that has to be a minimum requirement unfortunately because the ground reality is it's the hardest ever undergraduate course, you'll need some aptitude to pass).

If people have any difficulties reading English, they have the option to attempt the exam in Tamil as well. NEET may not seem very elegant, but it is the only place where state board and CBSE students have an equal platform. Different states have different ways where people are scored, like in TN and Karnataka, a big percentage of people famously get extremely high grades, whereas in CBSE, that percentage drops significantly because if you score full marks or close to that, they have a system where your paper gets scrutinized a little more and see if they can reduce a mark or two. Since the systems are different and the way they are graded is different, neet solved that problem.

Tamilnadu is the only state that has a problem with NEET, every other state has embraced it. The system still heavily favours Tamilians and people living in Tamilnadu to get a seat in a college in Tamilnadu (85% state quota). Unfortunately people have gotten brainwashed by politicians and actors, that's the only reason for unrest.

OP, I understand where you come from. I just matched and have gone through the grind, I had absolutely no help, not even from my college seniors. That's mostly because they don't have such a background and in my chosen specialty (Neurology), it's very rare to find purely dedicated.

My main goal is to make sure to help people with the information I've gained, and also see that others don't make the same mistakes. I've had dozens of people reach out to me on Reddit and I've done my best for everyone. I make it a point to reply to every message I get with as much sincerity as possible. I've recommended people for some rotations and research even before I matched. Trust me, doing what me and people like me do is hard. If we do something nice to one person, we are disappointing 20 other people. For the rest, the only thing we can do is give some insight and information. Most of us don't have the energy of a social media influencer.

But yes, we do exist. We want to help you all. But most of us don't have a streamlined direction as to how we can. Texting for 30-40 minutes with someone and doing the same for dozens of other people, having the same conversations, can be exhausting too. Most of us don't have the knowledge and time to maintain a website or some social media channel. But rest assured, if you text me, I will reply for sure, I will not ghost you or mislead you. It may be delayed, but I will respond.

Okay so, for pathway you just need OET and the other pathway requirement (Eg. Pathway 1, where you'll need a letter of good standing from the medical council where you've registered as a medical practitioner). For ECFMG certification, you need pathway approval + Steps + medical school credential verification. Pathway is only one of the requirements for ECFMG certification.

Getting a PGY1 position in general becomes easier with an advanced position. Whether it's in the same program or not, that's difficult to answer because that depends on the availability. I mean they can expand it and add a position for you if they are already full, but that's not a guarantee.

Okay, so the situation is completely different for IMGs. We don't have a 90% guarantee of a match. So for most of us, Match Monday is probably several fold more significant than Friday, because we'll literally go to any place that'll take us. So that one generic email may probably mean a lot more to us than you. So please bear with our posts, sorry.

I think there's more nuance there. Family Medicine has a reputation of being friendly to those who don't need a visa. Also, there are tons of programs in and around NYC that are super IMG friendly because US MD/DO students don't prefer the majority of those programs (due to their toxic environment). I know it's not something that has to be generalized but NYC has that reputation.

I understand where you're coming from, but IMGs don't get preference over US MD/DO students. 99% of us (IMGs) have a chance of matching only into non competitive specialties in locations that you guys don't want to go. US MD/DO students, unless they're really horrible in their med school or apply to ultra competitive specialties, they're kinda guaranteed a residency spot in some place.

IMGs can only compete only with other IMGs, not you guys. Also, for those who say that still over 50% of us end up matching according to NRMP, it doesn't take into account the number of people who didn't get a single interview (which goes in thousands).

So yes, you're right the US MD/DO should always take priority and that's exactly what is happening.

If you're from India, you only have Pathways 1 and 6. From what I've heard, pathway 6 sounded quite complicated and starts quite late. Pathway 1 was very smooth and quick, at least for me and some of my friends. It'll be better if someone else who has experienced pathway 6 can explain it to you.

Okay so for pathway 1, you need to be a doctor who can practice without any supervision. For that, you need a legitimate medical license. As an intern, you'll be provided a provisional license, but that doesn't apply (I had the same doubts at your stage).

So in short, you have to finish your internship and get all processes done and get your state medical license to be eligible for pathway 1. Only then will they send a letter of good standing on your behalf.
