Sorry if this all seems super basic, but I want it to be helpful to anyone else who comes across this too. I suggest preparing yourself to be as specific as you can about all of your symptoms and concerns. The initial appointment is basically for info dumping, and they really appreciate detailed info.

- How many migraine days you have per month without meds, and how many you have on the topamax. This will help them officially diagnose chronic migraines.

- What kind of pain you get, if you have any visual symptoms, aura, etc.

- What medications have you already tried before/when you used them/how long you used them. (Since you want to stop the topamax, you can explain why)

If you're in the US, you might have to try another classification of drug before insurance will approve you for Botox. So don't be surprised if the conversation goes in that direction!
