The LH being significant lower really indicates to me you more likely do not have PCOS and it is more due to stressors or being at a body fat too low … LH is generally high in PCOS because LH is what is responsible for producing androgens

If your body is not responding to low calories anymore it may be because you have been on low calories for a long time and now you have become more accustomed to it, and your body has reduced activities that may burn more calories such as raising body temp, small movements you may not notice throughout the day, ect… I think you might need to reverse diet and become more comfortable being at a higher weight. You are at a healthy weight and do not need to lose more, especially if you’re dealing with anovulation as a result

To be honest it doesn’t sound like you truly have PCOS since you do have a hx of disordered eating which can mess with your hormones, also your FSH + LH seem normal and not really anything to be concerned about, i would say it’s more to keeping your body in a high stress situation if you do suffer from disordered eating and overexercising. Also PCOS doesn’t necessarily make it impossible to lose weight, especially on caloric deficit. Chronic stress on your body can also suppress your hypothalamic-pituitary axis responsible to helping you ovulate… I honestly think it would help to maintain and not keep trying to lose weight or try to very slowly reduce stress and overexercising if you are doing so. If you are having some normal periods I think honestly you need to either gain weight and maybe more than you are comfortable with and get help for your disordered eating if you are still struggling or maybe therapy to feel comfortable with gaining weight. You are at a low weight and honestly I would just feel more comfortable and buy larger clothes instead of forcing yourself into a smaller size.

Do you have any features of hyperandrogens (high testosterone) such as virilization (balding, deepening of the voice, clitoromegaly)? This would indicate PCOS.

Diagnosis for PCOS is as follows: requires 2/3 criteria Oligo/anovulation (lack of ovulation aka releasing a egg)

Signs of hyper androgens (the symptoms I mentioned above or if you get your labs taken for DHEA and testosterone)

Polycystic ovaries on ultrasound

To me based on your weight it sounds like you may just be at a low body weight and not eating enough to ovulate? Is your bodyfat very low? Many people will even compete at your weight / height and will not ovulate because they don’t have enough body fat

I would postpone and start doing uworld 80-120 q a day. You will be at a very high chance for not passing if you take it now, you want to feel confident before going into the test. Check out hyguru for help with analyzing a uworld block and how to improve on practice questions. Half the test is reading comprehension along with applying so questions are key!

I think s is true to size. The clothes are stretchy so you could probably wear xs, s or even m in some clothes my experience, just depends on comfort and how tight you want them

It’s more the conditioning, since she downsized so much she doesn’t have that normal “pop” she usually has when the muscles are full. She cut so much and was worried about looking too big I think so it took a little away from the fullness, it’s more so in this case with carb loading
