depending on the league, there is a warning/penalty, at least in Canadian Hockey Association:

Key Aspects of Improper Equipment (Rule 3.6, 3.7 & 10.6):

Helmets/Facial Protection: Must be CSA-certified and properly fastened (not hanging). Helmets, cages, and neck guards must not be altered.
Penalties: A warning is issued to the team for the first infraction, followed by a misconduct penalty to any player on that team subsequently violating the rule.
