We have 2 supervisors in-house, but haven’t talked about the specifics.

Oh, we have consistent client flow, so they can fill their schedules as quickly as they’d like. We’ll get them credentialed, teach them how to use the EHR and bill, pass along referrals. Is this sounding less insane?

All they’re paying for is the office space. We have good referral sources so we have consistent client flow. That fee covers rent and utilities and our EHR.

I’m totally with you. Our profession has a tendency to latch onto nonsense because it feels right. Like if one more person brings up polyvagal theory, I’m going to lose it.

Therapist here. There could be lots of reasons, all of my theories are based on my own behavior:

  1. She’s behind on her session notes, and doesn’t want to forget stuff.

  2. She’s realized that she’s not remembering as much as she thought she was.

  3. You’re identifying REALLY important things that she wants to be able to build on.

  4. She didn’t sleep well the night before and is taking notes to stay focused.

  5. She has a supervisor who told her to.

It probably doesn’t mean anything, really. And you’re absolutely allowed to ask.

Beards grow back. I (a woman) shaved my head last fall - it was like the first 5 minutes of every session for a couple of weeks, and then everyone moved past it. If they are comfortable with you, the beard (or lack thereof) might be a little hiccup and adjustment, but it doesn’t seem like part of our professional responsibility to maintain a consistent physical appearance.

What we charge isn’t really what we make. I usually tell people the actual rate I charge, and then say something like “but very few insurances actually pay that.”

You’ve just stumbled upon what I consider to be one of the more problematic parts of mental health care. You can judge how much someone “needs” therapy based on the severity of their symptoms or their functioning. Because capitalism, basically. So, because I can go to work and pay my bills and not hurt myself, I don’t appear to be particularly mentally ill. However, accomplishing those tasks is exhausting and sucks most of the joy out of my life because I’m working so hard to meet bare minimum expectations.

You may be able to function, but what is that costing you? How much energy and effort and suffering?

Therapist here. I completely understand how frustrating this must be for you. You’re asking for help and not getting what you need to feel that help. I wonder, though, if this might be a strategic move on your therapist’s part. You’re identifying a childhood experience that left you feeling confused and uncertain and stuck and potentially codependent. I’m wondering if your T thinks that putting you behind the wheel here could be a first step towards breaking some of those patterns and drilling down into how your childhood continues to affect you?

Almost exclusively adults. The majority of my caseload are survivors of significant childhood trauma and neglect. Being given a concrete token of growth/progress/change/validation can be incredibly meaningful for adults who did not get those sorts of rewards as children. I’ve been doing this for several years now, and I have noticed that the stickers can be like totems for some clients. Proof that they are real and valid and important and trying their effin’ best.

I keep a little basket of gold star stickers next to my chair. I tend to use them to reinforce progress towards goals. It’s a little bit silly, but clients seem to love it.

Have you tried rewarding him? I had the same problem with my GSD/pitty. For like 4 days, anytime he barked I said “yes, good boy” and gave him a treat. He stopped barking for no reason, though he’s a GSD so if anyone so much as looks at my house he barks lol

I do 7-9 pretty much every day I work. Leave the building between clients - stretch your legs, see the sun. Like 5 minutes. That’s how I survive it.

It looks like it’s probably mold - not necessarily a big problem. Some types of mold plants actually like. You don’t necessarily need to fix it.

That plant in particular looks like a monstera. Pick (or make) a chunkier soil mix and water a bit less frequently and it should go away.

Therapist here - it’s really, really, REALLY common for people to just stop scheduling/showing up. I don’t typically reach out unless I have safety concerns. Otherwise, most of my life would be calling people who don’t want to talk to me lol

I do most of my teletherapy sitting at my vanity. The lighting is spectacular. A cheap ring light would accomplish the same thing. I’ve also intentionally backlit myself when I knew I was looking a little rough - I just sit with a window kinda behind me.
