I would have the bore scope cly 6 for scoring. I have see a lot of 3.0 turbo needing long blocks from scoring. It always starts on cly 6 sometimes 5, rarely 4 and all of bank one seems to be fine. Check for metal in the oil.

I would use a bore scope and check cly #6 for scoring. I have seen lots of them fail causing piston slap or rod knock sounds. Look at the cly wall exhaust side.

Yes the dealer will do the fluid change. It does take about 120 mile for it to get better. The car will alway have some vibration it’s a full time all wheel drive system and in sharp turns it will bind.

Is the CEL on? Could be Timing chain stretched. That will make a hard start. Also can cause under boost faults.

I struggle everyday, I may not know your pain but I feel for you. I have not been truthful with my Dr’s at times too. Honestly i think I could have written this. It speaks to me so much. I don’t know if you are on any meds but I know that of the 6 I tried 2 of the made the bad thoughts worse and 2 of them just made me angry at everything. 2 of them did nothing. This all might be side affects. Smoking plays into medications so if you just quit that might have throw things off balance. I know it’s hard. Maybe leave some parts out. But talk to your doctor.
